Objective: Insulin resistance plays a pivotal role in the development of type 2 diabetes mellitus. Diabetic vascular complications is the major reason leading to death in patients with type 2 diabetes.The levels of plasma von Willebrand factor(vWF) is reported to an marker of endothelial damage/dysfunction. It is associated with increased the events of diabetic vascular complications. We measured plasma levels of von Willebrand factor in patients with type 2 diabetes. To elucidate the relationship between decreased insulin resistance and the effect of endothelial function.Methods: We measured plasma von Willebrand factor levels, by enzyme-linked immunesorbent assays , blood pressure,body mass index, waist-to-hip ratio, glycaemia, insulin concentrations, hemoglobin (Alc), insulin resistance index (HOMA-IR) and lipid metabolic parameters before and after treatment of rosiglitazone in 104 patients with type 2 diabetes mellitus.Results: (1) The blood pressure, glycaemia, hemoglobin (Alc), fasting insulin concentrations, the levels of HOMA-IR and vWF were significantly decreased after treatment (P<0. 05). (2) HOMA-IR were highly correlated with body mass index,waist circumference, hip circumference, waist-to-hip ratio, fasting plasma glucose, insulin concentrations and systolic blood pressure. The plasma vWF concentrations were positively correlated with age and creatinine. (3) Multivariate analysis: vWF were correlated with triglyceride, fasting plasma glucose, postprandial plasma glucose,diastolic blood pressure and age before treatment. vWF were correlated with age after treatment. The effect factors on vWF range of change were Alow density lipoprotein and A endogenous creatinine clearance rate. HOMA-IR were correlated with fasting insulin concentrations and fasting plasma glucose. The effect factors on HOMA-IR range of change were A fasting insulin concentrations and A fasting plasma glucose.Conclusion: The effect of improve the insulin sensitivity not only can decrease the blood pressure, glycaemia, insulin concentrations and vWF , but also can improve to treat vascular complications with type 2 diabetes mellitus by rosiglitazone.
