IntroductionObesity means energy intaking exceed energy expending in a long - terms, results in much energy storing with fatty form in body, and injures human health. Simple children obesity is some kind of life behavior factors which lead to obesity. 95% children obesity belongs to simple obesity . Obesity, as risk factors of adult hypertension, hyperlipemia, atherosclerosis, heart angiopathy, diabetes and a great variety of cancers , has already become common disease which severely harm to children health. The obesity rate in chinese cities exceeds much than developed countries , this increasing trend has become mainly children health problems in Cities . With the increasing numbers of obesity children, the disease development trends of children diabetes and cardiovascular are increasing.Many studies have showed recently higher correlation between obesity and insulin resistance (IR). Adipose tissue plays important role in mechanism of IR, Adipose tissue secret some lipocyte factors as leptin, lipoprotein lipase , tumor necrosis factor - Alpha , interleukin - six, and resistin , they all show higher relationship with obesity . Now animal and adult research show resistin relates closly with obesity and insulin resist, it may participate in attack of obesity and obesity - related II diabetes , but so far , the study of obese children serumresistin is little and conclusions are various . In this study, the levels of children serum resistin, leptin, insulin, blood glucose were measured in obesity group, normal group and thinness body weight group, differences are compared in three groups about levels of resistin, leptin, insulin, blood glucose , and relation to insulin resist , in order to supply theory ideas about prevention and Control children obesity and complication.Objectives1. Investigating levels of FPGAserum insulin , resistin, leptin changes in obese children.2. Investigating whether insulin ,resistin, leptin associated with obesity and obese - related HOMA - IR.Objects and Methods1. Investigating objects ?.Children 1835 joining physical examination were selected in Shenyang certain region , according to" WHO height and body weight commendatory standards :For young - aged children (sexual equality) " ,exceed 120% standard for obesity;80% -120% for normal;Under 80% for thinness body weight,90 samples were screened out , age is three -four years. 30 in obesity group ,30 in normal body weight group , and 30 in thinness body weight group, male and female is 1: 1.2. Investigating methods2. 1 Physical checking: Both height and body weight measurement follow Chinese body constitution report of investigation.2.2 Serum insulin and leptin concentrations were assayed with radioimmunoassay kits, FPG levels were assayed with ELISA method, and blood glucose were assayed with 7170s Automatic biochemistry analyzer.2. 3 Samples collecting;5 ml venous blood samples were gained in all groups, part of whole blood samples were checked for blood glucose , other bloodsamples were centrifuged and the serum was stored at -2.4 HOMA IR=FPG(mmol/L)XFINS^IU/L)/22.5. 2. 5 Statistical method;All analyses were carried out using SPSS 11.5 statistical software. All variables are expressed as means s. Data in different sex were analyzed by Students t - test, levels of FPG, FINS, Resistin, HOMA - IR were analyzed by ANOVA, Correlation was tested using Pearson'method.Results1. Height and weight of obesity, normal children and leanWeights of obesity children were significantly greater than lean and normal groups ( p <0. 01) . But height of the obesity children did not differ from those of the lean and the control groups, (p >0. 05) .2. The level of serum leptin, FPG, resistin and H0MA_IR in boys and girls Serum leptin concentrations were higher in girls than in boys(p <0. 01).There was no significant difference between girls and boys in FPG , serum leptin, resistin and H0MA_IR.3. Serum leptin, FPG, FINS, resistin and H0MA_IR in obesity, lean and normal childrenFPG , FINS , leptin and HOMA _ IR concentrations, as expected, were higher in obesity children than in lean and normal groups(p <0. 01). And FPG , FINS, leptin andHOMA_IR concentrations were higher in normal children than in lean children( p <0. 01) . No significant differences were found in resistin expression in obesity children , lean and normal children (p >0.05).4 . The relation between FPG, serum leptin, FINS, resistin and HOMA_IR Significant positive correlation between FPG and leptin, FINS, H0MA_IR (P<0.01). And significant positive correlation between leptin and FINS , HO-MA_IR( P <0. 01). Significant positive correlation between insulin and HOMA_ IR(P<0.01). But there was no correlation between resistin and FPG, serum leptin , FINS , HOMAJR.DiscussionsObesity is a chronic wasting disordered condition that the energy of intake is more than the utilization. In our country, obesity now has been to an important public health problem involving the children.1. The relationship between obesity and leptin NIRFor the past few years, researches show that most obesity accompanied IR. The mechanism of IR caused by obesity is not clear. This investigation shows that the values of FPGAFINSNHOMA - IR in obesity were obviously higher than the other two groups, although the values FPG were normal in obesity. All this indicates that maybe the IR occurs in obesity. IR is a dominant feature of 2 type diabetes mellitus disease. Our results showed that the diabetes mellitus disease increased in obesity group. The mechanism of the lipid metabolic disorder caused by IR correlating with obesity may be (1) the enzymatic activity of LPL was weakened (2) the enzymatic activity of CETP was enhanced. The insulin has the effect to promote lipogenesis, so obesity would aggravate with the increase of insulin. This is a vicious cycle.This research showed that the girls had higher values of leptin compared with the boys and the sexual difference of the children is according with the a-dults. The conclusion is that the level of sex hormone and the distribution can greatly influence leptin, especially the relation of sex and leptin, somebody thought it was partially made by the difference of women's peripheral fat distribution.Some reports showed that leptin has the effect to restrain or to promote the insulin secretion. Maybe leptin plays a certain contribution to the forming process of IR, because there is much closer relationship between leptin and IR. When leptin and leptin - receptor cohered in nucleus arcuatus hypothalami, the insulin level will be cut down with the decreased of NPY secretion, however, the insulin secretion stimulated by glucose was restained when leptin cohered with his receptor in the beta Cell of islet. It was discovered in animals that insulin was an important factor, which could regulate the expression of leptin - receptorin hypothalamus. The leptin - receptor gene mutation leaded to bonding obstacle, which destroyed the leptin signal transduction passageway. All this resulted in the leptin resistance condition surrounding tissues and destroyed the feedback regulation system of " fat - insulin" axis. So the insulin resistance took place. This research indicated that there was a significant dependability between leptin and insulin resistance index and coincided with the reports above - mentioned.2. The relation of resistin and IRIn 2000, America scientist Steppar discoveried a kind of hormone secreted by fat cell, it can resist insulin and increase blood glucose,named resistin. At the same time, he found insulin may be an important joint of fat and the IR. Many articles indicated: the relations between resistance and obesity, IR are close. Insistin can inhibit adipocyte grow to mature, which having greatly relation with obesity;it can also inhibit the glucose's utility mediated by insulin, which leads to the increasing of blood glucose. Some researches on primarily hypertension adult patient, who is different condition of carbohydrate tolerance discovered : the blood - serum resistin significantly increased with the damage of carbohydrate tolerance, which was correlated positively with the level of blood glucose. But there are some contradict ideas. It cues that the expression of animal and humans tissues is difference, which was effected by means of paracrine or autocrine, but the indeed mechanism in human is not clearly.The result of this research indicates: there are not obvious differences between the resistin levels of three childrens, the fat children' s FINS level is significantly correlated with the weight, BMI, FPG, insulin, and HOMA - IR, it indicates: we should deeply study the relations of resistin, the metabolism of fat children' s sugar and IR.Conclusions1. Serum leptin^ FINS levels increased in obese children. Serum resistin level had no change.2. Positive correlation between leptin and HOMA - IR was found;No correlation was found between resistin and leptin NFINS ^HOMA IR.
