Background: Lichen planus(LP) is a chronic or subacute inflammationdermatopathy,its sickness rate is about 0.5 %-1% in the crowd.The skinlesion is often mauve,polygonal,flat papula, and common oral mucosaldamage. The etiological factor and pathogenesy has not yet been finalized.CD44 is a kind of transmembrane glycoprotein that is on distributed cellsurfacein of lymphocyte and mechanocyte, is a cell adhesion molecule.Current domestic and foreign study found that T Cell-mediated immunereaction participates the morbidity's course of LP, and CD44 molecule canenhance the property of T lymphocytic activation and multiplication, therefore,we speculate that CD44 molecule may participate lymphocytic infiltration andactivation of LP, leading to the LP pathological changes.Objective: The study of the expression of CD44 molecule among theseskin lesion of LP,and the discussion of the relation about CD44 molecule andlymphocytic infiltration,are to find the etiopathogenesis of LP.Methods: The experiment group selects the skin lesions of 30 LP patientswhose final diagnosis is through clinic and biopsy.The control group selects 10norml skins and 30 skin lesions of these dermopathies with lymphocyticinfiltration in derma layer(such as: granuloma fungoides, Sézary syndrome,dermatitis glandularis erythematosa, ad.), then we initiate the study of CD44through imunohistochemistry,observe the condition of dyeing and count.Countmaterials adopt Fisher examine. If P<0.05, it is significant on statistics .Results: 1. CD44 molecules express in epiderm layer both norml skinsand skin lesions of LP.They express on the surface of heckle cells and basalcells, neither granular layer cells nor corneous layer cells,and express thestrongest on heckle cells.There is no obvious difference about expression ofCD44 molecules on the surface of heckle cells and basal cells between normlskins and skin lesions of LP(P﹥0.05).2. CD44 molecules do not express in dermis layer of normlskins.Lymphocytes are band-like infiltration in superior dermis layer of LP,and CD44 molecule's expression is obviously fortified. In these dermopathieswith lymphocytic infiltration are no obvious increase of CD44. There aresignificant differences in them(P<0.01).Discussion: Currently many immunology research think that TCell-mediated immune reaction participates the morbidity's course of LP. Theexperiments confirm that the expression of CD44 is strong masculine onlymphocytic surface in dermis layer of skin lesions of LP and no obviousincrease in these dermopathies with lymphocytic infiltration. Therefore, wecan speculate that CD44 molecule possesses significant sense in lymphocyticinfiltration and activation of LP.Because CD44 can combine with extracellularmatrix(such as: fibro-ancylo-protein, collagen protein, etc.),that leads tolymphocytes of CD44 expression combine with endothelial cells of highendothelial venules and gather toward inflammation part.we can think thatCD44 participates lymphocyte homing toward inflammation part of skin,andinduces lymphocytic band-like infiltration in dermis layer.In the course oflymphocyte-action, the expression of CD44 on T-cells increases temporarily.CD44 molecules act the "costimulatory molecules" and combine withhyaluronic acid, then cause T lymphocytic activation and multiplication withTCR/CD3 complex receptive excitation. In addition, Siegelman fond that therewere large CD11a expression on the surface of infiltrative lymphocytes,andT-cells can correlate with CD3 + T through the cooperation CD44 andCD11a,that leads to T-cell-stimulating. These facts show that CD44 moleculepossesses significant sense in lymphocytic infiltration and activation.After T cells are activated,they excrete many cytokines(such as: IL-2,IFN-r,GM-CSF). IFN-r can induce that malpighian cell expressesabnormal antigen,as HLA-DR and ICAM-1, that course further attractsT-cells,and make T-cells stick with malpighian cells of abnormal surfaceantigen,then kill malpighian cells directly or indirectly. In addition, activatedmacrophages can excrete many cytokines,they participate histiooytic damagecourse of LP. This series of reactions leading the basal cells to damage causeLP pathological changes.Conclusion: 1.There is no obvious difference about expression of CD44molecules in the epiderm layer between norml skins and skin lesions of LP.2. There is obviouse enhancement on the lymphocytic surface in dermis layerof skin lesions of LP .3.CD44 molecule maybe participate lymphocytic infiltration andactivation of LP, leading to the band-like infiltration in superior dermis layerof LP,and liquifaction degeneration of basal cells.
