Objectives: Lichen planus (LP) is a chronic, subacute,inflammatory Cutaneous disease reported first by ErasmusWilson in 1869. It occurs most often in men and womenbetween 30 and 60 years, and uncommon in the very youngand elderly. In crowd, incidence of this disease is lower than1%, and its tendency of morbidity heightens at present. Thetypical lesion of this disease appears as shiny, flat-toppedpapule that frequently have any angular shape withreddish-purplish color. LP can occur anywhere on the skin, butoften favors limbs rather than trunk. This disease frequentlycompanion oral mucosa lesion, and often occurs on buccalmucosa. The ulcer lesion maybe have a tendency ofcarcinomatous change. The histopathology of LP showedhyperkeratosis, anomalous hypergranulosis and acanthusis,liquefaction degeneration of basal cells, and infiltration likeband by lymphocytes in superficial lamella of dermis. Therehas been a controversy as to the origin of LP and nosatisfactory therapy for this disease in recent years. At presentstudies have hinted that LP is induced by an immunologicalmechanism associated with T cell-mediated immunity and the 4英 文 摘 要results of some immunocyte examination were abnormal.Immunological function of T lymphocytes depended on theinteractions of various cytokines and antigen presenting cells(APC, Langerhans cells, LC, etc.). The studies on Tlymphocytes and correlated cytokines and LC in LP have notbeen reported detailed in civil and abroad. Moreover, it's hardto reveal all mechanism of LP only by abnormality of Tlymphocyte. So this study was designed to explore thecorrelation between the Immunological function of Tlymphocytes in peripheral blood and expressions of TNF-α,IL-12p40, CD1a, HLA-DR in the lesions so as to illuminate thepathogenesis of LP, to observe other possibility of morbidit ofLP, and to offer elementary theory for the clinical treatment ofLP. Methods: Thirty-five LP patients, diagnosed by clinic andhistopathology, were collected from the dermatologicaldepartment of the Fourth Affiliated Hospital of Hebei MedicalUniversity from June, 2002 to June, 2003. The patientsincluded 23 males and 12 females in this study, who had noother systemic diseases and had not received anyimmunotherapy during 3 recent months; age ranged from 11 to73 years (mean 37.26±16.40 years); course from 20 days to 20years (mean 26.4±19.68 months). The lesion samples wereinvestigated with routine histopathological methods, the levelsof TNF-α, IL-12p40, CD1a, HLA-DR expressions weredetected by immunohistochemical method. Twenty normal skin 5英 文 摘 要specimens were taken as normal controls collected fromnormal skin of trauma patients without any other disease indepartment of surgical and osteology of the Third AffiliatedHospital of Hebei Medical University. There was no significantdifference in sexes, ages and locus of biopsy between controlsand patients. Selecting randomly twenty-one of 35 patients for4 millilitres peripheral blood samples in morning was to use forlymphocyte transplant test, included 13 males and 8 females,age ranged from 15 to 77 years (mean 35.14±16.43 years);course from 20 days to 14 months (mean 5.46±3.39 months).Adopting 11 normal individuals as controls and there was nosignificant difference in sexes and ages between controls andpatients. Apply SPSS11.0 statistical software handlings data.Paraffin section was used by HE staining in histopathology andby histostain-SP method in immunohistochemistry. Lympho-cyte transplant test was used to measure stimulation index (SI)of peripheral blood. The above results were analysed by ranksum test and Student t test , respectively. Results: 1.
