Objective:To compare the treatment results of interlocking intramedullary nail andexternal fixation in open tibia and fibula fractures retrospectively. Investigatethe operation indication ,ideas,complication of interlocking intramedullary nailand external fixation in open tibia and fibula fracturesDocuments and Methods:The 297 cases of were selected from January 2000 to February 2006 inthe China-Japan Union hospital of Jinlin Universicity. They are of open tibiaand fibula fracture .The patients' ages were between 18 and 60. they arebellowed type IlIa according to Gustilo, 171cases were treated withexternal fixation;126 cases were treated with interlocking intramedullarynail,all were followed-up for 6—18 months.To compare healing time, operation indication ,ideas and complication inboth groups, the occurrence of complications.The statistical analysis was performed by SPSS 11.0.ResultAll the cases were followed-up for 6 一 18 months. Results 171cases weretreated with external fixation;the rate of excellence and good were 85.7%,126 cases were treated with interlocking intramedullary nail,the rate ofexcellence and good were 70%, To compare the rate of excellence and good,they have significant difference (P<0.05). Th healing time of thenailed group is lower than the external fixed group. To compare the healingtime , they have significant difference (P<0.05). The nailed group haveadvantage over the external fixed group as follows:lower rate of infection,delayed union and nonunion,excellence with Johner-Wruh evaluation,shorttime of healing,and they have significant difference (P<0.05).Conclusion:Compared with external fixation , the treatment of interlockingintramedullary nail has the characteristics of stability,few complications andhigh rate of fracture recovery,it is more suitable for the treatment of belowGustilo 1IIa open tibia and fibula fracture,especially for many segmentfracture. The treatment of external fixation is simple and it has thecharacteristics of few injury,which can be applied effectively in treatment ofCustilo IIIb open fracture and nearby joint fracture in tibia and fibula.It cansupply chance for soft tissue recovery.
