Objective : To study the correlation between power Doppler imaging and microvessel density ,vascular endothelial growth factor about breast masses angiogensis and to valuate the clinical application.Materials and methods: 55 patients with breast mass (25 benign, 30 malignant) were performed by power Doppler ultrasound. The features of blood flow in neoplasm were evaluated by power Doppler ultrasound. Meanwile, the peak systolic velocity , resistive index were recorded by pulsed Doppler spectrum. Patterns of vascular, color pixel density with computer-assistant of power Doppler flow signal were analyzed. The microvessel density and vascular endothelial growth factor was measured in surgical specimen after immunohistochemical staining using anti-factor CD34, VEGF and compared to color pixel density.Results: The blood stream signal in malignant tumor was rich than benign tumor. The blood stream signal in breast carcinoma was mostly grade III, but the blood stream signal of grade II has no statistical significant in the differentiation of malignant and benign breast masse (P>0.05) . Center, penetrating vascular structure has more significance in malignant lesions than benign lesions. The peak systolic velocity and resistive index of vascular structures were calculated by Doppler spectral, resistive index was highly in malignant breast masses than in benign breast masses (P<0.05), The peak systolic velocity had no significance (P>0.05) . Color pixel density and microvessel density was greater in malignant group than in benign group(P<0.01). Axillary node-positive tumors demonstrated significantly higher color pixel density and microvessel density than did axillary node-negative tumors(P< 0.01). These studies showed a good correlation between color pixel density by power Doppler imaging and microvessel density by histologic examination(r=0.846, r=0.889, P<0.01). Vascular endothelial growth factor was greater in malignant group than in benign group(P< 0.01), Vascular endothelial growth factor were higher in the tumors with metastasis than those without metastasis(P<0.01). Both color pixel density and microvessel density was significantly higher in VEGF-positive breast carcinoma than in VEGF-negative breast carcinoma.
