Breast Conservative Therapy For Breast Cancer: A Retrospective Cohort Study

Objective: Breast cancer is one kind of malignancy that seriously threaten to women's health. With the deeper recognization of it's pathogenesy, we find out that breast cancer is a systemic disease just perform at local place. The extensive surgery can not improve the prognosis. In developed countries, instead of modified radical mastectomy, breast conserving therapy is the standard therapy in women with early stage breast malignancy. We use a matched retrospective cohort study to compare the effects of breast canservation therapy (BCT) with modified radical mastectomy (MRM) in women with early stage breast cancer.Methods: A matched retrospective cohort study using data on patients derived from a prospectively collected breast cancer database was conducted The database included patients who received MRM or BCT from 2002 to 2006 in Firsr Hospital of Jilin University.The match was conducted according to four variables: dimension of tumor, axillary lymph node status, sexual homone receptor and age at diagnosis. The match ratio was 1:2. Cases were patients who received BCT (n=35). Control group were patients who received MRM (n=70). Median follow-up for the controls and cases were 35 months and 16 months respectively. The differences of disease free survival and overall survival were compared. And we also analysis the cosmetic result.Results: There are no patient local recur in BCT group and one in MRM group. There are one distant-metastasis patient in BCT group and five in MRM group. Four patients were dead of breast cancer metastasis, one in BCT group, the others in MRM group. The OS and DFS in MRM and BCT patients have no difference (p>0.05). According to JCRT's standard we evaluate the postoperative cosmetic appearances, the satisfation rate is 88.57% (31/35).Conclusion: For appropriate breast maligancy patients, classic breast conserving surgery plus post-operative radiatherapy can result in the same effects as modified radical mastectomy in breast malignancy patients, but BCT can not only receive better cosmetic appearances but also improve the quality of life.
