Heart failure is one of the most complicated clinic syndrome. Recently, the morbidity and mortality of some common cardiovascular diseases has decreased, however, that of heart failure grows increasingly meanwhile. Heart failure is the chief complication of all organic heart diseases caused by totally different etiological factors. In the last few years, a lot of documents indicated that inflammatory factors and oxidative stress play a significant role in causing heart failure. Uric acid (UA) is the end products of purine metabolism, which is usually generated by the promotion of xanthine oxidase, in which process superoxide anion and free radical are also produced. Uric acid production and metabolic pathway may play a significant role in chronic congestive heart failure physiology and clinical picture.Objective: to study the relationship between the level of blood uric acid and the damage it caused to heart function and renal function by detect blood uric acid(UA)and ultrasound parameters(LVEF,E/A),Blood urea nitrogen(BUN)and creatinine(Cr) of the patients with heat failure.Methods: One hundred and fifty-two patient with heart failure . 58patients ranging in class II, 46 in class III, 48 in class IV. to detect the level of blood uric acid(UA)and Blood urea nitrogen (BUN)and creatinine(Cr).Patients underwent echocardiography -Dopler examinations. Mitral E wave and mitral A wave relocities. (E/A)ratio. left ventricular ejection fraction (LVEF) . We try to explore the relationship between blood uric acid and blood urea nitrogen,creatinine through the method above. In this study we divide the objects into two groups, one with a blood uric acid level exceeding 420uμmol/L and one with a level less than 420uμmol/L. At the same time, divide into two groups with blood urea nitrogen level in 7.0mmol/L.Results: 1 as the heart function aggravating LVEF decreases and the level of UA increases gradually. There is a negative correlation between LVEF and UA,also between UA and E/A. 2 when the level of blood uric acid go higher, that of Blood creatinine(Cr)augments also. Two sets BUN compare do not statistically significant. The UA level increase higher in BUN≥7.0mmol/L. 3 with the class of heart function increase , BUN and Cr increase too .Discussion: Leyva reported that the level of blood uric acid is closely related with that of the blood inflammatory index of patient suffering from heart failure and it reflect that body's oxidative metabolism has been damaged. It is reported that a high level of blood uric acid is one independent risk factor that leads to death from congestive heart failure and influences the prognosis of patients with coronary heart disease, with a direct correlation with case fatality of coronary heart disease . During heart failure, reduction of blood flow in kidney, glomerular filtration rate cut down. Aronson D indicated, the level of Blood urea nitrogen increase can prognosticate case fatality of coronary heart disease.Conclusion: Elevated uric acid levels are associated with CHF. there is a closely relationship between blood uric acid level and contraction and diastolic function . hyperuricacidemia is one independent risk factor that leads to death from cardiovascular disease and influences the prognosis of patients with congestive heart failure . BUA can be used as a anticipation index for the aggravation of heart function. Harm to renal function can also be used to estimate the prognosis of patients with heart failure. Blood uric acid is correlated with renal function after precluding the effect renal has on blood uric acid. Xanthine oxydase inhibition might theoretically result in an improvement of heart function.
