| Now, the pathogenesis of chronic sinusitis and nasal polyp disease are not complete known. Recently, as the developpment of a lot of experimental skills, more and more study approve that Nitric oxide (NO) and inducible isoform nitric oxide synthase (iNOS) play an important role in the pathogenesis and developpment of chronic sinusitis and nasal polyp disease. NO takes part in regulations of circulatory system, respiratory system, digestive system and nervous system. NO is a very small airy molecule. It play an important role in inflammation and immune reponse. NO is synthetised by nitric oxide synthase. Thereare three type of nitric oxide synthase (NOS): neuronal nitric oxide synthase (nNOS), inducible isofoun nitric oxide synthase (iNOS) and endothelial nitric oxide synthase (eNOS). nNOS and eNOS that are called structural nitric oxide synthase (cNOS) mainly regulate physiologyical. The treatment of chronic sinusitis has four objectives: to control infection, to reduce tissue edema, to facilitate the drainage of sinus secretions and to maintain ostial permeability. Medical treatment includes antibiotics, decongestants, mucolytics, steroids and analgesics. Surgical treatment may be conservative, functional or radical. At present,"FESS"is the"Golden Standard". It is focused on the middle ostiomeatal complex and the ethmoid cells. So the blockade of sinonasal drainage by anatomical abnormalities (like paradoxicity of middle turbinate, e. g. convexity facing laterally; concha bullosa, e. g. aircells in the head of middle turbinate, variation in the uncinate process and large ethmoid bulla) will predispose to different sinus conditions like inflammatory and polyp pathology due to stenosis in middle meatus. So endoscopic endonasal surgery of diseased ethmoid is important in treatment of chronic sinusitis. Endoscopic sinus surgery has enjoyed impressive success curing chronic disease in sinuses. With the introduction of nasal endoscopy, functional endoscopic sinus surgery (FESS) has become an important means in treating chronic sinusitis and polyps. But the surgery merely is in a whole treatment link important step, the surgery end is following closely is the mucous membraneepidermis regenerates and restores the stage.The experiment observed the therapeutic effect of Bang Bi-jing irrigation solution and the expression of inducible isoform of nitric oxide synthase (iNOS) after functional endoscopic sinus surgery (FESS) for chronic sinusitis (CS) and nasal ployps(NP).Using All the sixty CSâ…¡cases were randomly classed into two groups:the test group and controlled group, every group contained 30 patients. The patients of the test group were treated with Bang Bi-jing irrigation solution to irrigate the operating sinus cavity, once a day. The controlled group were treated with 0.9%N.S.we followed up all the patients after 12 weeks and recorded the changes of symptoms and signa of patients. The expression of iNOS were studied by means of immunohistochemistry. The result wereâ‘ The symptom of two groups had significant difference (the symptom of test group was 93.3%, the symptom of controlled group was 76.7%).â‘¡The total effective rate of two groups had statistical significance: the total effective rate of the test group was 93.3% and that of the controlled group was 86.7%.â‘¢The negative rate of the iNOS expression had significant difference: the negative rate of the iNOS expression in test group was 60% and that in the controlled group was 16.7%.We could get the conclusion which were:â‘ The detection of iNOS expression in NP after FESS can be considered as a reference to the objective standard of operation effect and a risk factor to predict the NP recurrence.â‘¡Bang Bi-jing could accerlate the recovery of mucous membrane by regulating iNOS in NP after FESS.â‘¢The use of Bang Bi-jing irrigation solution after FESS would mitigate symptoms and signs of patients and improve the curative effect.
