Objective To compare serum high sensitivity C-reactive protein(hs-CRP)levels and evaluate relationship between serum high sensitivity C-reactive protein(hs-CRP)and severity of coronary arterial lesion in patients with coronary heart disease(CHD)with varying glucose tolerance.Methods 125 patients with positive angiography measure disk factors,the Gensini' s score was used to determine the results of the coronary angiography.All subjects in the study underwent a 75g oral glucose tolerance test(OGTT).They were divided into four groups:30 cases in simple coronary heart disease group(G1),35 cases in CHD with abnormal glucose metabolism group(G2),35 cases in CHD with type 2 diabetes mellitus(T2DM)group(G3)and 25 cases in normal control group(G4).All patients measured hs-CRP level,fasting insulin(FINS), homestasis model assessment for insulin resistance(HOMA-IR),blood total cholesterol(TC),blood triglyceride(TG),low density lipoprotein cholesterol(LDL),high density lipoprotein cholesterol(HDL),apoaA level,apoaB level,blood uric acid(BUN)and body mass index(BMI)and the relationship of the severity of coronary arterial lesion was observed at the same time.Results①The serum hs-CRP levels of the four groups were significantly higher than those of the normal controls group(P<0.01);the concentrations were highered in the simple coronary heart disease group,CHD with abnormal glucose metabolism group and CHD with type 2 diabetes mellitus group(P<0.05).②The Gensini's score differed among groups(P<0.01).③The serum levels of hs-CRP were positively correlated with the Gensini's score,FPG,2hPG,HOMA-IR,BMI,TCand TG,hs-CRP was independent associated with the Gensini's score.④NO significant differences in the incidences of multi-vessel lesions and the severity of coronary arterial lesions among CHD with abnormal glucose metabolism group and CHD with type 2 diabetes mellitus group,serum hs-CRP concertration were increased sequentially by different number of branch lesion,hs-CRP were associnated with the Gensini's score(P<0.05).Conclusion Higher level of hs-CRP is associated with increased severity of coronary atherosclerosis with T2DM,the incidences of multi-vessel lesions and the severity of coronary arterial lesions were significantly high in CHD with abnormal glucose metabolism.The serum levels of hs-CRP were significantly high in CHD with abnormal glucose metabolism,the hs-CRP levels were closely positively correlated with severity of coronary lesion and may play a role in proceeding and developing of coronary artery disease,It suggestes that serum hs-CRP levels can be parameter for prognosticating the severity of coronary atherosclerosis in patients with abnormal glucose metabolism.
