Objective:1,To observe the changes of the serum concentration of soluble intercellular adhesion molecule-1 (sICAM-1) soluble CD40 ligand (sCD40L), before and after PCI( percutaneous coronary intervention) in patients with different types of the coronary artery disease, explore the pathogenesis of coronary heart disease and the changes of sICAM-1 and sCD40L after PCI .2,To determine the relationship between sICAM-1 and sCD40L in different types of coronary heart disease(CHD).Method:According the results of CAG(coronary angiography),69 patients were enrolled ,22 type A CHD patients with PCI ,type B 26 CHD patients with PCI ,,21 persons whose CAG was normal act as control group. Plasma concentrations of sICAM-1 and sCD40L were measured by enzyme-linked immunosorbent assay(ELISA). Serial venous blood samples were obtained 1 day before and 1 day,5 day after PCI .Results:1,There were no differences between the serum concentration of sICAM-1 in type A group and type B group as determined (P>0.05) before PCI;The concentration of sICAM-1 in type A and type B group was higher when compared to those found in control group before PCI(p<0.05) .After PCI sICAM-1 of type A and type B group was significantly higher than before PCI (p<0.05) .The gradually increased trend was followed by time after PCI. There was no change in control group(p>0.05) after PCI.2,The Serum concentration of sCD40L in type A group and type B group was higher when compared to those found in control group before PCI(p<0.05).;There were no differences between the serum concentration of sCD40L in type A group and type B group as determined (P>0.05) before PCI; After PCI sCD40L of type A and type B group was higher than group(p<0.05) .After PCI 1 day the increase was the most prominence ,and after 5 day the level of sCD40L was still higher .3,The sCD40L concentration was correlated with sICAM-1 (r=0. 467, P<0.01).Conclusion:The increase of sICAM-1and sCD40L in patients of CHD suggest that it may promote the triggering and propagation of CHD. The increases of sICAM-1and sCD40L after PCI are correlated to the factors of crushing atherosclerosis like balloon inflation,in-stent and so on ,revascular after coronary injuring and reflow is another reason .The increases of sICAM-1and sCD40L are correlated to the degree of atherosclerotic plaques.
