| Objective: Hemorrhoids is one of the most common diseases in clinic. Surgical intervention is the main treatment of choice for theⅢ,Ⅳdegree hemorrhoids. The Milligan-Morgan hemorrhoidectomy is currently a most popular surgery of hemorrhoids. Due to the particular anatomy of anus, postoperative pain is often terrible, and the pain may come as a chief postoperative complication, which would delay the time to recover to work or have an impack on the quality of life. There are many methods of pain killing after Milligan-Morgan hemorrhoidectomy, but all of them have more or less some shortcomings. For a new analgesia which is cheap, highly effective and easy to manipulate normatively, we would prefer Abdominal Acupuncture. Methods: Our study included 63 patients undergoing Milligan-Morgan hemorrhoidectomy use subarachnoid space block anesthesia between April 2006 and April 2007, who were randomly assigned into two groups: a test group (Abdominal Acupuncture group, 31 patients) and control group (Tramadol injection group, 32 patients). The test group were treated with Abdominal Acupuncture, for 15~30min, and the control group with Tramadol injection 0.1g i.m. when analgesia was asked by the patients. The VAS scores and body activity scores were recorded before treatment and 0.5h,1h,2h and 3h after treatment. The adverse effect were recorded simultaneously. All data were analyzed by SPSS 12.0.Results: Since no significant difference existed between them in the following respects:①the age and sex of the patients,②the degree of hemorrhoids,③the number of incision,④the number of hemorrhoids,⑤the time consumed by the operation, as well as⑥the time elapsing after the operation. VAS and body activity scores were also similar before treatment. The two groups were comparable. The only significant difference was observed in VAS scores at 0.5h after treatment, with 2.8065±1.40046 in the test group vs. 3.6563±1.57827 in the control group, p<0.05. The dose of Tramadol used in the test group has been 0.013±0.0341, as compared to 0.103±0.0177 in the control group, p<0.05.Conclusion: The effects of Abdominal Acupuncture was better than Tramadol injection 0.5h after treatment. Though these two in treatments appeared similarly at the other observed times, abdominal acupuncture has yet another merits of lower cost, fewer adverse reactions, and easy to manipulate. Abdominal Acupuncture is a new alternative therapy of Postoperative Pain after Milligan-Morgan Hemorrhoidectomy. |
