Evaluation On Effect Of Developing And Applying Appropriate Health Care Technology In The Rural Areas Of Henan Province

AimWith medical science's grad progresses, Today we are sharing top-grade medical treatments, but have to face some serious health problems too, like higher medical care charges, more unadvisable medical resources distribution, the normal treatment's have no progress and so on. To solve these problems, the most important way is that Appropriate Health Care Technology must be developed along with modern medical technology. Appropriate health care technology was narrowly defined to be the kinds of diagnostic, medical, and surgical technologies-procedures as well as equipment and the systems necessary to support them-which might be used in small and medium sized hospitals, clinics, or mobile units serving large populations. Especially in rural, developing and applying Appropriate technology is very useful to solve the major health problems of villagers. It can improve the village medical service level, improve the health care of peasants, and reduce the peasant's medical charges.1991,The Ministry of Health of R.P.C began to practise the plan of proffer one hundred appropriate technologies in the rural areas within ten years. Since then, they have selected and proffered 152 appropriate heath care technologies, have trained 3,5000 medical skilled workers. Acquired great achievements on Social behalf. 2004,Ministry of Science and Technology of P.R.C cooperating with Ministry of Health of R.P.C, State Administration of Traditional Chinese Medicine of P.R.C, National Commission of Population and Family Planning, carry out the study on the development and applying of appropriate Health care technology, one of National Key Technologies R&D programs, science Research for the tenth Five-Year Plan. We applied for this program, and at the end of 2004,we are agreed to take part in it. So we undertook the study on the development and applying appropriate health care technology in rural of HeNan province. Find some ways to evaluate synthetically the effect of development and using Appropriate Technology on village public health, the government can take some advice from the project's experiences to establish scientific objective, practical and effective indicator system to promote the development of appropriate health care technology in rural areas of China. Materials and methods1.Object: The objects were administrative system and running mechanism of appropriate technology study.2.Variables: Independent variables were source, sum and proportion of the finance, management and running of the project, health service index of the Appropriate Technology. Dependent variables were effect of appropriate health care technology.3.Methods: According to the theory of Donabedian, the administrative father of America medical system, appropriate indexes were used to evaluate the effect of appropriate health care technology from three aspects: framework, process and outcome. (1) Systematic method was used to assessed the framework of appropriate health care technology. (2) Description the outcome of Appropriate Technology's selecting, applying, and training medical staff. Using RSR to evaluate the need of Appropriate Technology, help to select technologies. (3) According to the aim of applying appropriate technology, select 12 indices and ivied them into dieses treatment level, treated patient amount, economic and social behalf. Using TOPSIS to evaluate synthetically the effect of development and using Appropriate Technology on the village level health care.4.Statistics: Used soft ware of Foxpro 6.0 and SAS 6.12.Results1 .The spots of study are representative of mid-developed countries of Henan, Their economic situation and health care standards are better, and have the ability to applying Appropriate Technology.2.The program of training is finished. Hold short courses for 1841 doctors, nurses and technicians. 88.38% of them are familiarity with the operation of appropriate technology. The proportions of higher educating and higher-profession are bigger than the common level of china.3.2154 patients accept the treatment by appropriate technologies. On the average, 135 patients per technology treat, 1.23 case per technician treat. The level of country is higher than the village's.4.Ether in ChangGe country and Mengzhou country, The values of synthetic evaluation in 2006 are higher than that in the 2005,the level of dieses treatment level, treated patient amount, economic and social behalf improve significantly. 14 in the 16 appropriate technologies, their values of synthetic evaluation in 2006 are higher than that in the 2005. 2 appropriate technologies' values have no obvious distinction between 2006 and 2005 year, but the value is near 0.5.Conclusions1. Administrative system and funds raise system running fine. 2.The program of training and applying appropriate health care technology is finished efficiently. Improve the village medical treatment level, health service utilization of the appropriate health care technology. The level of dieses treatment level, treated patient amount, economic and social behalf improve significantly. The running mechanism provided not only a rudiment for the perfection of applying appropriate health care technology, but also a reference mode for the perfection and foundation of rural health care system.3.
