Objectives To assess the current situation of rural doctors’human resource in impoverished areas. To confirm the situation and influence factors of primary health care training in impoverished areas. To evaluate the effect of primary health care training in impoverished areas.Methods One group pretest-posttest design was used in this study. Using a convenience sampling method, selected the rural doctors who attended primary health care training as the research object in impoverished areas of Yanbian. A total of141questionnaires were inssued,123questionnaires were usable. The research tool based on "Rural health personnel training program guidance" which promulgated by ministry of health of the People’s Republic. After consulting foreign and domestic literatures, based on "the outline of rural doctors on-the-training" in Jilin Province. All collected data were inputted into computer and analyzed by SPSS for Windows16.0.Results1.The present situation of rural doctorsThe investigated rural doctors were predominantly male, average age was46.46±11.66,the clinical work was more than31years, the medical degree of rural doctors was secondary education, professional rank was primary, similar to the national level. The knowledge of primary health care(PHC) mainly came from books, regular trainings. The urgent require training contents were clinical practice, traditional Chinese medicine, the primary health care knowledge about rural areas.2.The one-way ANON of knowledge, attitude, behavior, demand of PHCWith the one-way ANON, the scores of the PHC knowledge were significant different in age, length of clinical work as a rural doctor, degree of education; The scores of the PHC attitude were significant different in income, professional rank; The scores of the PHC behavior were significant different in length of clinical work, degree of education, professional rank; The scores of the PHC demand were significant different in different age, length of clinical work, degree of education, professional rank.3. The multiple linear regression of knowledge, attitude, behavior, demand of PHCWith the multiple linear regression, the scores of PHC knowledge were influenced by the degree of education, sex, demand; The scores of PHC attitude were influenced by professional rank and demand; The scores of PHC behavior were influenced by professional rank and length of clinical work; The scores of PHC demand were influenced by the scores of knowledge, length of clinical work and the scores of attitude.4.The effect of PHC training of rural doctorsThe score of PHC was12.60±2.69,after intervention improved to16.95±2.75,and statistically significant(P<0.01).The scores of prevention of infections diseases, disaster response and emergency, hypertension, diabetes prevention and management were higher than pretest, and statistically significant(P<0.01).Pre-posttest the disaster response and emergency changed obviously (t=-17.62,P<0.01),the prevention of infections diseases changed not obviously (t=-3.09,P<0.01)ConclusionsThe level of age, degree of education, professional rank of rural doctors were similar to the national level, but the knowledge of PHC was poor, in the future, the training should be based on the needs of the rural doctors and the actual condition of each district. Training could improve rural doctors’PHC knowledge level, and the urgent needs could guardant the feasibility and continuity of on-the-job training. Should established the scientific and reasonable on-the-job training system, Improved the overall quality of the rural doctors.
