Objective: To investigate the pathological features of esophageal cancer and gastric cancer patients after operation in the Fourth Hospital of Heibei Medical University, and analyze the epidemiology of the esophageal cancer and in Hebei Province. Methods: Selecting randomly 7251 cases of esophageal cancer and gastric cancer in the Fourth Hospital of Heibei Medical University from Jan 1996 to Dec 2005. All data collected was analyzed by a SAS 6.0 software package withχ2,t test and linear regression. A statistically significant difference was indicated by a P<0.05.Results:1 Esophageal cancer1.1 The changes of the clinical correlated characteristics in the last 10 yearsAge: the mean age was 57.1 years, which has been raising over the past 10 years.Predilection site: the constituent ratio of the superior, intermediate, or inferior segment have not changed obviously. Pathological type: the constituent ratio of the squamous esophageal cancer has decreased (P<0.05), and the constituent ratio of the oat cell carcinoma has rose(P<0.05). The constituent ratio of other types have not markedly changed.( P>0.05).Soakage depth: In all, the constituent ratio have not markedly changed, but the ratio of infiltrative depth to the muscular coat has decreased( P<0.05).Clinical stages: the constituent ratio of stageⅡhas decreased(P<0.05). The ratio of stageⅣhas rose over the past 10 years(P<0.05).Resection methods: the ratio of the patients undergoing radical cure, palliative operation and thoracotomy have not markedly changed(P>0.05).1.2 Referring to gender, the mean age of female patients was later than male. The ratio of lymph node metastasis and advanced stage patients in female was fewer compared to male(P<0.05).There was not significant difference between female and male in the constituent ratio of pathological types(P>0.05).1.3 In the view of age group, there is not marked changes in the ratio of gender in the three age group(P<0.05). the ratio of the adenocarcinoma patients in the elder group is bigger than its ratio in the middle and youth group(P>0.05).1.4 As to predilection site, the ratio of gender, age, pathological type, infiltrative depth, clinical stage and lymph node metastasis were statistically different in the three segments of esophageal cancer ( P<0.05).Compared with superior and intermediate segments , the elder patients and adenocarcinoma accounted more in the inferior segment, while female accounted fewer in the this part. The ratio of the patients with lymph node metastasis, infiltrative depth to fibrous membrane, and stageⅢ,stageⅣaccounted more and more from the superior to the inferior.1.5 The length of affection was positively correlated with the infiltrative depth and TNM stage.2 Gastric cadia cancer2.1 The changes of the clinical correlated characteristics in the last 10 yearsAge: the mean age was 59.9 years, which has been raising over the past 10 years.Pathological type: the constituent ratio of the mucinous adenocarcinoma has decreased (P<0.05), and the constituent ratio of the signet-ring cell carcinoma has rose(P<0.05). Soakage depth: but the ratio of infiltrative depth to the muscular coat has decreased, the ratio of infiltrative depth to the surrounding soft tissue has decreased ( P<0.05).Clinical stages: the constituent ratio of stageⅡhas decreased(P<0.05). The ratio of stageⅣhas rose over the past 10 years(P<0.05).Resection methods: The ratio of the patients undergoing thoracotomy have decreased(P>0.05).2.2 Referring to gender, the mean age of female patients was before than male(P<0.05).There was not significant difference between female and male in the constituent ratio of lymph node metastasis and advanced stage patients(P>0.05).2.3 In the view of age group, there is not mark change in the pathological category and stage in the youth, middle age and older age group(P>0.05).2.4 The length of affection was positively correlated with the infiltrative depth and TNM stage.Conclusions:1. Esophageal cancerThe mean age of the esophageal cancer patients operated in the Fourth Hospital of Heibei Medical University has rose over the past ten years. The constituent ratio of theⅡstage patients has decreased. The constituent ratio of theⅣstage patients has rose over the past ten years. The mean age of female patients was later than male, the ratio of lymph node metastasis and advanced stage patients in female was fewer compared to male(P<0.05).The ratio of the adenocarcinoma patients in the elder group is bigger than its ratio in the middle and youth group The elder patients and adenocarcinoma accounted more in the inferior segment, compared with superior and intermediate segments , while female accounted fewer in the this part. The ratio of the patients with lymph node metastasis, infiltrative depth to fibrous membrane, and stageⅢ,stageⅣaccounted more and more from the superior to the inferior. The length of affection was positively correlated with the infiltrative depth and TNM stage.2. Gastric cadia cancerThe mean age of the esophageal cancer patients operated in the Fourth Hospital of Heibei Medical University has rose over the past ten years. The constituent ratio of theⅡstage patients has decreased. The constituent ratio of theⅣstage patients has rose over the past ten years. The ratio of the patients undergoing thoracotomy have decreasedThe mean age of female patients was before than male. The length of affection was positively correlated with the infiltrative depth and TNM stage.
