Objective: To study the dangerous factors leading to the canceration of colorectal polyps through the retrospective analysis of materials about patients with colorectal polyps confirmed by colonoscopy and pathology check.Methods: 529 clinical patients which were out-patient clinic or in hospital from 2000 to 2006 were checked with colonoscopy.There were 761 polyps we find there were 43 patients and 53 polyps with carcinomatous change in total confirmed by the pathology check. We respectively stasised the canceration number according to gender,age, the size of the polyps, single or multiple polyps, the position of polyps ,the pathohistology type of polyps and analyzed the results with the chi square test.Results: (1) The carceration rates of men and woman are 7.8% and 9.0% respectively,there are no significantly statistical difference(P=0.69).(2) the carceration rat of patients whose age are old than 40 years is higher than the less than 40 years patients, there are significantly statistical difference between them(P<0.007). (3) With the developing of the intestine size,the carceration rate has increased,there are significantly statistical difference between carceration rates of the large than 1.0cm and the less than 1.0cm(P<0.0125).(4)The multiple polyps has higher canceration rate(17.5%) than the single polyps (4.8%), there are significantly statistical difference between them(P<0.05).(5) The polyps which grow in rectum or sigmoid colon has higher canceration rate than in ascending,transverse and descending(P<0.005).(6) The adenoma and especially the villous type adenoma has high canceration rate than others, there are statistical significance between of them(P<0.0125).Conclusion: (1) The sex is not one of the dangerous factors leading to the canceration of colorectal polyps ,the dangerous factors are the old than 40 years,the polyps intestine size large than 1.0cm,the multiple polyps,the polyps which grow in rectum or sigmoid colon,the adenoma and especially the villous type adenoma.(2)The most way in diagnosing and preventing canceration of colorectal polyps is colonoscopy。(3)We must develop the magnification,chromoscopic or ultrasound colonoscopy as soon as possible。(4)We need to strengthen the propaganda and enhance the consciousness of the high risk people. |
