Objective: To discss photocoagulation effect for diabetic retinopathy patients dring stage 3 to stage 5 as well as diabetic macular edema patients ranged from different sorts . Method: Different stage was given to diabetic retinopathy according to international classification proposed by the 2002 American association of ophthalmology (AAO), while ETDRS standard was chosen for diabetic macular edema ranking. All of patients were recorded as follows: best corrected vision before and at least 6 months after photocoagulation, fundus examination and fluoresec in fundus angiography (FFA) inspection. Therapetic effect was judged on the basis of vision criterion and FFA standard, then statistic analysis was carried out.Result: For the matter of vision condition, stage 3 and 4 of nonproliferative diabeticretinopathy (NPDR) had difference from stage 5 of proliferative diabeticretinopathy (PDR) on the aspect of photocoagulation, with the former having better effect than the latter. Along with DR lesion progressing, eyes turned to decrease in vision improvement, while ratio of eyes with vision lowering appeared to increase. According to FFA standard, stage 3 and 4 had different therapeutic effect from stage 5, with the former having better effect than the latter. Regardless of vision condition or FFA, therapeutic effect difference existed for different sorts. that was, diabetic focal macular edema patients had best curative effect. While accompanied by disease degredation, ineffective eyes gradually increased and stabilized eyes turned to decrease no matter whether in vision criterion or in FFA standard.Conclusion: On the hand of DR lesion, stage 3 and 4 of nonproliferative diabeticretinopathy (NPDR) had better therapetic effect than stage 5 of PDR on the aspect of photocoagulation, so prompt treatment shold be taken. Specifically, for stage 3 patients, milddisseminated photocoagulatin could be applied, as for stage 4 and stand 5, standard panphotocoagulation treatment might be better. For DME lesion, according to ETDRS standard, diabetic focal macular edema had better effect compared to diffuse edema and cystoid edema .In the concrete, for diabetic focal macular edema , focal photocoagulation might be good, but referring to diffuse edema and cystoid edema,"C"grid photocoagulation shold be chosen.
