Objective To investigate the clinical and pathological characteristics,diagnosis and treatment of gastric cancer with ovarian metastasis. Methods Retrospectively analyzed the clinical data of 18 cases of gastric cancer with ovarian metastasis that were confirmed by surgery and pathology in our hospital between 2002.1.1-2007.12.12. Results The incidence of this disease was 2.4% (18/750) .The median age of the patients was 42.0 years (30-72 years) .16 of them were pre-menopause which was 88.9%(16/18) of all, and 16 of them used abdominal pain or abdominal distention as their chief complaint. The main ultrasonographic findings in the ovary were either a complex type of mass with both solid and cystic characteristics or only solid , or sometimes homogen low echo-level. 11 patients' pathology pattern were signet-ring cell carcinoma, 3 were poorly differentiated adenocarcinoma, 3 were moderately-poorly differentiated adenocarcinoma, 1 was poorly differentiated adenocarcinoma accompanied with signet-ring cell carcinoma. The misdiagnostic rate was 44.4% (8/18) in this series. Operation was done in all patients, and 13 of them were given chemotherapy. The follow-up time was 5month~ 66month, median survival time was 7.2months, and one year survival rate was 20%. Conclusions Although the incidence of gastric cancer with ovarian metastasis is low, we should look out to it in young women, especially whose gastric carcinoma was in sinus ventriculi and corpus ventriculi, pathology pattern were signet-ring cell carcinoma, and Bormann type were III-IV. The prognosis of this disease is poor, operation is the first choice, Chemotherapy may improve the prognosis.
