Objective(1)To explore the expression of vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF),tumor necrosis factor-alpha(TNF-α)and the changes of microvessle density in the development of esophageal varices in patient with portal hypertension.(2)To explore the expression of vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF)and the changes of microvessle density in the development of hypersplenism in patient with portal hypertension.Methods specimens of esophageal(30 cases)from portal hypertension patients underwent modified Sugiura procedure and specimens of spleen(30 cases)from portal hypertension patients underwent modified Sugiura procedure served as experiment groups.Normal esophageal tissue(10 cases)and splenic tissue(10 cases)served as the control group.The expression of VEGF,bFGF and TNF-αin esophagus and splenic tissue was studied quantitatively by immunohistochemistry SP method and computer assisted image analysis system.SPSS13.0 statistical package was used to analyze relationship between the expression of VEGF,bFGF and TNF-αprotiein and the clinicopathological feature in esophagus and spleen in patients with PHTResults(1)The expression of VEGF was mainly seen in the mucosal layer in esophageal tissue and in the splenic tissue.PHT groups>control groups(p<0.05).The expression of VEGF in esophagus and spleen in patients with PHT was not related to Child-pugh and bleeding of esophageal mucous before operation(P>0.05).(2)The expression of bFGF was mainly seen in the mucosal layer in esophageal tissue and in the splenic tissue.PHT groups>control groups(p<0.05).The expression of VEGF in esophagus and spleen in patients with PHT was not related to Child-pugh and bleeding of esophageal mucous before operation(P>0.05).(3)There was morderately positive correlation between the expression of VEGF and bFGF in esophageal tissue tissue(r=4.37 P=0.029)and splenic tissue(r=4.47 P=0.041).(4)The expression of TNF-αwas mainly seen in the mucosal layer in esophageal tissue.PHT group>control group(p<0.05).The expression of TNF-αin esophagus in patients with PHT was not related to Child-pugh and bleeding of esophageal mucous before operation(P>0.05).(5)Regions of high microvessle density mainly located in the mucosal laminal propria and submucosa of esophageal and splenic tissue in patients with portal hypertension.The MVD of esophageal tissue and splenic tissue was significantly different between portal hypertension group and control group(P<0.05).The MVD of esophageal tissue and splenic tissue was not related to Child-pugh and bleeding of esophageal mucous before operation(P>0.05).Conclusion(1)The increase of the expression of VEGF and bFGF in esophageal tissue in patients with PHT and the MVD of esophageal tissue indicate that VEGF and bFGF was related to the development of esophageal varices(2)The increase of the expression of VEGF and bFGF in splenic tissue in patients with PHT and the MVD of splenic tissue indicate that VEGF and bFGF were related to the development of hypersplenism(3)There was morderately positive correlation between the expression of VEGF and bFGF in esophageal tissue and splenic tissue.It suggest that there was synergism between VEGF and bFGF in development of esophageal varices and hypersplenism(4) The increase of the expression of TNF-αin esophageal tissue in patients with PHT indicate that TNF-αmay induce injury of esophageal mucous...
