Objective:Adiponectin, a novel circulating anti-inflammatory protein made by adipocytes, has been reported to be lower in diabetic than nondiabetic subjects. We sought to investigate the relationship between the serum adiponectin and urinary albumin to creatinine ratio (UACR), body mass index (BMI), blood lipid, blood pressure, fasting plasma glucose (FPG), glycated hemoglobin A1c (HbAlc) and C-peptide in type 2 diabetic patients with different stages of nephropathy.Methods: Data on 61 type 2 diabetic patients from the department of endocrinology and 30 normal healthy participants were analyzed. According to UACR, patients were divided into three groups: normal albuminura group (UACR<30mg/g, n=24), microalbuminura group (UACR=30~299mg/g, n=22), macroalbuminura group (UACR≥300mg/g, n=15). Serum adiponectin concentrations were measured in frozen sera (samples were drawn between June and September 2007) by ELISA using a commercial reagent set. The intra- and interassay coefficients of variation were 7.8% and 9.5%, respectively. HbAlc and C-peptide of these patients were also measured. BMI, blood lipid, blood pressure, FPG of all objects were measured. All statistical analyses were performed using SPSS version 11.0 for Windows(Statistical Package for Social Science, USA). Nonnormally distributed variables such as adiponectin and FPG were ln-transformed to better approximate normal distributions.Results: The level of serum adiponectin was higher in normal healthy people than type 2 diabetic patients and was higher in microalbuminura group than normal albuminura group, and the serum adiponectin concentration in the macroalbuminura group was significantly higher than normal and microalbuminura groups. By Spearman correlation, serum adiponectin was positively correlated with UACR (r=0.769, P<0.001) and inversely correlated with BMI (r=-0.543, P =0.001), TC(r=-0.565, P<0.001), fasting C-peptide(r=-0.639, P<0.001) in 2 type diabetic nephropathy patients. Multivariate linear regression analyses showed the main influence factors were UACR, BMI and fasting C-peptide.Conclusion:With the increase of UACR, the level of serum adiponectin increased.
