Preliminary Study On The Relationship Between Psychology Status, Nutrition Status, Tumor Markers In Peripheral Blood And The Life Quality Of Digestive Cancer Patients

ObjectiveTo investigate the expressing information of the psychology status, nutrition status and tumor marker--circulating DNA and serum CEA in peripheral blood of digestive tract cancer patients, to analyze the influencing factors of the quality of life in digestive tract cancer patients, and to provide the basis for rehabilitation therapy.MethodsOur sample contented 85 digestive tract cancer patients who were postoperative about 6 to 12 months in the Fujian Cancer Hospital. The cases were surveyed by trained interviewers with constructed questionnaires. Linear regression model was used to analyze the quantitative relationship of digestive tract cancer patients between nutritional status, psychology status and quality of life. Cumulative odds logistic regression was used to analyze the influencing factors of quality of life, and to calculate odds ratio(OR) and 95% confidence interval(C.I.). Meanwhile, to collect the peripheral blood of investigation objects, patients on disease of digestive tract and people of health examination. Real-time florescence quantitative polymerase chain reaction(FQ-PCR) used for circulating DNA in the plasma, electrogenerated chemilumines cence(ECL) used for serum CEA. One-way analysis of variance was used to compare the cases and the control group in the circulating DNA and serum CEA. All of the data analyze was performed with the SPSS version 13.0 and SAS8.2. Results1. The incidence rate of malnutrition was high among digestive tract cancer. the low rate of protein was 47.06%, the shortage of heat was 42.39%, and the limitation of haemoglobin was 31.76%.2. The incidence rate of anxiety and depression was high in the Psychology status of digestive tract cancers, with 45.86% and 31.76%, respectively. There is a close line-to-line relationship between anxiety and depression ,with linearχ2=23.452, P = 0.001.The more anxiety, the worse depression among patients.3. The circulating DNA in the plasma and serum CEA of postoperative patients with digestive tract cancers were higher than those in control groups. The level of circulating DNA in the plasma are different between case group and the control group(P <0.001),there is no difference between benign case and the healthy control group (P=0.528). Serum CEA are different between case group and the healthy control group(P <0.001);there is no difference between benign case and the control group (P=0.674)4. Digestive tract cancer postoperative patients had linear correlation between nutrition and quality of life, psychological status and quality of life. The more nutrition status, the worse quality of life among patients. The more psychological status, the worse quality of life among patients. The expression of circulating DNA in the plasma and serum CEA are the difference statistically significant on the quality of life in different levels.5. Cumulative odds logistic regression analysis suggests that the main factors of effect the quality of life are age(OR=1.653,95%C.I:1.434~1.979), appetite(OR=0.490,95%C.I:0.305~0.787) and staging of disease(OR=2.228,95%C.I:1.277~4.214).6. As a result of the quality of life variables, multivariate linear regression equation to the variables are depression(X1), appetite(X2) and age(X3). The best multivariate linear regression equation is Y=71.486+0.416 X1-2.907X2+0.210X3. Conclusions1. The incidence rates of malnutrition are very high in digestive tract cancer patients.The main manifestation were the low rate of protein, the shortage of heat and the limitation of haemoglobin.2. The incidence rates of anxiety and depression are very high in digestive tract cancer patients. There is a close line-to-line relationship between anxiety and depression. The more anxiety, the worse depression among patients.3. Digestive tract cancer postoperative patients had negative linear correlation between nutrition and quality of life, psychological status and quality of life. The main factors of effect the quality of life are age, appetiteand staging of disease.4. The levels of circulating DNA in the plasma and serum CEA in digestive tract cancer postoperative patients were significantly higher than that of control, The levels of circulating DNA in the plasma and serum CEA in digestive tract cancer postoperative patients were different among quality of life.To detect the levels of circulating DNA in the plasma and serum CEA in digestive tract cancer postoperative patients was very significant in evaluation of quality of life .
