| Objective: To analyze the clinical characteristics and the clinical application values of video-assisted thoracoscopic surgery in the diagnosis and therapy of solitary pulmonary nodule(SPN).Methods: From January 2005 to December 2008,the clinical data of 121 patients with SPN who had received video-assisted thoracoscopic surgery were retrospectively analyzed. All cases with SPN were performed the pulmonary wedge resection by Endo-GIA follow by frozen pathologic check. The cases with a malignant diagnosis were treated further. Compare the diameter of SPN and to determine wether the location of SPN related to the quality of the lesions; To determine wether the diameter of the malignant SPN related to the local lymphatic metastasis.Result: 58 cases were diagnosed malignant(47.93%). There were no statistical significance between the diameter and location with the quality of SPN(P>0.05). There are statistical significance between the diameter of the malignant SPN with the local lymphatic metastasis(P<0.05). All the patients got definite pathological diagnosis and received operations successfully. There was not severe intraoperative and postoperative complication, and there was no perioperative death.Conclusion:1,The diameter and location cannot be used to differ the quality of the SPN(belign or malignancy). The diameter of the malignant node related to the local lymphatic metastasis.2,VATS is a new approach for the diagnosis and treatment of solitary pulmonary nodule,with less invasion but more safety and efficacy.3,The SPN should receive operation without needing a long-term observation, avoiding delay the condition of a patient. |
