Objective : To investigate the correlation of hepatic oval cell and primary hepatic carcinoma , in order to provide the evidence that the primary hepatic carcinoma has its source from the hepatic oval cells. Methods: We choose 44 cases , including 6 cases of healthy people (3 men, 3women), 7 cases of hemangioma of liver, (3 men, 4 women), 8 cases of intrahepatic bile duct stone (5 men, 3 women), 6 cases of hepatic cirrhosis (4 men, 2women), 8 cases of primary hepatic carcinoma (7 men, 1woman), 9 cases of primary hepatic carcinoma incorporation cirrhosis (7 men, 2 women). After operation, we take the liver of all people(the size of tissue about 1.0 cubic centimeters), then use the immunostaining method to measure the number of the cells which express the cytokeratins-7(Ck-7), cytokeratins 19(Ck-19), C-kit, cytokeratins-8/18(Ck-8/18) , alpha-fetoprotein(AFP). According to different background, pathology diagnosis (benign or malignant,merger liver cirrhosis or not ),all cases are divided into different group again, using statistical software SPSS 13.0 software used for statistical analysis, all P <0.05 for the significant differences in the standards, discusses the difference between hepatic oval cells and primary hepatocellular carcinoma .Results: (1) the test group between gender, age, no significant difference (P > 0.05), with liver disease severity, the number of oval cells which express Ck-7, Ck-8/18, Ck-19, C-kit have increased trends;(2) primary liver cancer group, hepatic carcinoma merger cirrhosis group, posthepatitic cirrhosis, intrahepatic bile duct stone group , the number of hepatic oval cells which express Ck-7,Ck -8/18, Ck-19, C-kit with liver hemangioma, normal liver group is significantly (P<0.05). AFP only in primary liver cancer group, hepatic carcinoma merger cirrhosis group, its expression in the AFP respectively 62.5% (5/8) and 66.67% (6/9), but the expression of AFP has no statistically significant differences between the primary liver cancer group and hepatic carcinoma merger cirrhosis group;(3) according to whether the merger cirrhosis and liver diseases benign or malignant, we compared the group of benign liver group merger liver cirrhosis (intrahepatic biliary calculi group, posthepatitic cirrhosis group) malignant change group(hepatic carcinoma merger cirrhosis group), finding the number of positive cells which express Ck-7, Ck-8/18, Ck-19, AFP, C-kit has significantly difference between the two groups; the group of liver hemangioma compared with primary liver cancer group, there is significant differences about the positive hepatic oval cell express Ck-7, Ck-8/18, Ck-19, C-kit, AFP between these group. Therefore, no matter whether the liver merge cirrhosis, the hepatic oval cells in malignant liver express Ck-7 , Ck-8/18, Ck-19, C-kit, AFP have significantly difference with normal liver, benign diseases;(4) we compared malignant liver group(hepatic carcinoma merger cirrhosis group and primary liver cancer group), the expression of Ck-8/18, C-kit, AFP positive cell has no significant difference, but the expression of Ck-7, Ck-19, hepatic oval cells is significant difference; Comparison between benign liver groups, liver hemangioma group and benign liver cirrhosis (intrahepatic biliary calculi group, posthepatitic cirrhosis group), found no express AFP benign liver cell, the the number of cells which express Ck-7, Ck-8/18, Ck-19,C-kit has significant difference; (5) normal liver group and liver hemangioma group, there is a group of positive cells express C-kit in the liver hemangioma group, Both groups have expressd Ck-8/18 cells, the difference was not statistically significant, but the number of cells express Ck-7, Ck-19 has significant difference; (6) Compared with the normal liver group and hepatolithiasis group, there is a group of positive cells express C-kit in the intrahepatic biliary calculi group, both groups have expressed Ck-8/18 cells, but this was not statistically significant, and the number of positive cells express Ck-7, C-19, C-kit was significant difference;(7) Compared with the liver hemangioma group and the hepatolithiasis group, he number of positive cells express Ck-7, Ck- 8/18, Ck-19 has no significant difference, the expression of C-kit cells have significant difference;(8)liver cirrhosis group(hepatic carcinoma merger cirrhosis group, posthepatitic cirrhosis, intrahepatic bile duct stone group) , the expression of C-kit has no obvious difference in the benign liver group, but expressed significantly higher in the hepatic carcinoma merger cirrhosis group, there are significant differences between the three groups. Conclusions: (1) the expression of AFP positive in the cancer groups have significant difference with other group, there was a significant difference between benign and malignant liver group about the expression Ck-7,Ck-8/18, C-kit,Ck-19, in the growth of the tumour cell differentiation, regulation involve the hepatic oval cells;(2) liver stem cells is activated in the normal physiological condition, But when the normal liver cirrhosis process, more liver stem cells are activated, proliferation. The expression of Ck-7, Ck-19, C-kit in hepatic oval cells plays an important role when normal by development liver cirrhosis; (3) the hepatic oval cells express Ck-7, Ck-8/18, Ck-19, C-kit, AFP are significantly difference, by normal liver arrive to cirrhosis and primary liver cancer, the process of the hepatic oval cells in degree of activation, proliferation and may lead to a primary liver cancer,the expression of C-kit in the hepatic oval cells plays an important role in the process liver cirrhosis into primary liver cancer; (4) when the benign liver has converted to malignancy, the difference of the expression of C-kit in hepatic oval cells was not statistically significant. The found provides new ideas for the future study the pathophysiological mechanisms and liver stem cells for treating liver cancer.
