Objective: To detect the expression of B7-H1, B7-H3 and B7-H4 in the human pancreatic carcinoma tissue, and discuss the associations between the expression and clinical-pathological indexes and prognosis of the patients.Methods: The expression of B7-H1, B7-H3 and B7-H4 were measured in 32 cases of pancreatic carcinoma tissues, 10 cases of normal pancrea tissues by immunohistochemical streptavidin-perosidase technique. The pathologic and clinical data of these patients were also collected with futher follow-up. Futhermore, the relationships between the expressions of B7-H1, B7-H3, B7-H4 and clinical-pathological indexes and survival time were analyzed.Results: (1) Both the pancreatic carcinoma tissues and normal pancrea tissues were immunohistchemically stained with B7-H1, B7-H3 and B7-H4 antibody. The positive rates of B7-H1, B7-H3 and B7-H4 protein were much higher than that's of normal specimens (P<0.05). The positive staining for tumor specimens were mainly localized in the cell membrane, cytoplasm, or both. (2) The positive rates of B7-H1, B7-H3, B7-H4 protein in cancer tissues were 40.63%(13/32 cases), 90.63%(29/32 cases) and 75.00%(24/32 cases)respectively. The expression of B7-H1 protein was not associated with any clinical-pathological indexes (P>0.05). The expression of B7-H3 protein was significantly related to the tumor histological type(P<0.01), the pancreatic ductal adenocarcinoma and papillary mucinous carcinoma were much higher than the mucous cystadenocarcinoma of the pancreas. The positive rate of B7-H4 protein was associated with KPS score(P<0.01), the patients with a higher KPS score (>70) had a higher expression of B7-H4 than the patients with lower KPS score(≤70). (3) Multivariate Cox proportional hazards regression analysis found that theraputic regiment, the expression of B7-H1 and B7-H4 protein was correlated with the prognosis of these cancer patients(P<0.05), the adjuvant chemotherapy reduce the patients'death risk(β=-3.168,RR=0.042), the positive expression of B7-H1 and B7-H4 significantly increase the patients'death risk (βB7-H1=2.674, RRB7-H1=14.504;βB7-H4=1.480, RRB7-H4=4.395).Conclusion: The results suggested that over-expression of the B7-H1, B7-H3 and B7-H4 (immunohistochemistry analysis) were detected on the pancreatic cancer cells.The B7-H1 and B7-H4-positive expression had certain relation with the prognosis of the pancreatic cancer patients, and the combined examination of the two molecules might be useful in predicting the outcome of the pancreatic cancer patients.
