| Objective:By observing effects of probiotics on the plasma levels of DAO,D-lactate,IL-6,IL-10 in patients performed open-heart operation with cardiopulmonary bypass, the aim of our study is to show the protective effects and possible mechanism of probiotics on the intestinal mucosal barrier lesion induced by CBP. In addiction, our study provides experimental evidence for prevention and treatment of SIRS and intestinal mucosal barrier lesion induced by CBP.Methods:40 cases heart valve replacement bypass graft operations approved by Human Rights Commission were recruited randomly from Department of cardiovascular surgery, General Hospital of ShenYang Military Area Command, ASA II-III, aged 30-65 years, body mass 40-80 kg, and were diagnosed by no cure hypertension, cure anemia, liver and renal insufficiency, endocrine and neurological diseases. In addition, these cases have no symptoms of infection, coagulation disorders and abnormal left ventricular function before the operation. The 40 patients were divided randomly into 2 groups-probiotics group, control group and 20 cases a group:the probiotics group received Jinshuangqi (Bifid Triple Viable contain bifidobacterium, lacto-bacillus, Streptococcus thermopiles) four pills once time, three times each day seven days. Peripheral blood samples were collected at the time as follows:T1:before CPB; T2:10 min after CPB; T3:the endpoint of CPB; T4:2h after Operation; T5:6h after Operation; T6: 18h after Operation. Operation time, CPB time and residence time after ICU, Operation time and infection incidence are recorded. WBC in the blood are counted before and after the operation. the plasma levels of DAO, D-lactate, IL-6 and IL-10 are recorded. The results were measured by (x±s) and were analyzed with SPSS 15.0 software. Difference comparison of measurement data among groups were determined with Paired-samples T test, intra-group comparisons were determined with variance analysisResults:Two groups after the commencement of CPB, the plasma DAO activity, D-lactate density was higher than pre-CPB (P<0.05), and appeared first and then the process of reduction, peak appeared at two hours after Operation; the plasma DAO activity and D-lactate density of Y group were lower than C group after the commencement of CPB at different time points,and the difference of T2-5 time was statistically significant (P<0.05).C group and Y group after the commencement of CPB, the plasma density of IL-6,IL-10 was higher than pre-CPB (P<0.05), and appeared first and then the process of reduction, peak appeared at six hours after Operation; the plasma IL-6 density of Y group were lower than C group and IL-10 were higher than C group after the commencement of CPB at different time points, and the difference of T4-5 time was statistically significant (P<0.05).Conclusion:1. The decrease of the plasma levels of DAO and D-lactate indicates that probiotics has the protective function of intestinal mucosal barrier in patients with open heart surgery by Cardiopulmonary Bypass.2. The precondiction with probiotics could inhibit immune response after CPB. |
