Objective:To investigate the clinical characteristics of in-patient with hypertension and the use of antihypertensive drugs.Methods:Retrospective analysis was used to analyze the cases of hypertension with detailed medical record information in the department of cardiology in a unique third-level grade A hospital in Meizhou district in 1999 and 2008. The investigation includes the classification, complications and attainment rate of blood pressure ect. The using conditions of the five kinds of antihypertensive drugs (angiotensin-converting enzyme inhibitors, angiotensin II receptor blockers, calcium channel blockers,β-receptor blockers and diuretics) was analyzed.Results:1. Totally, there were 1036 cases of hypertension collected, of which there were 781 cases in 2008. In 1999 and 2008, the average age was (64.82±10.20) and (69.29±11.75) respectively, and sex ratio (m/f) was 1.13 and 2.17. During the ten years, the age of the patients and the number of male patients was increasing (p<0.01). As to the degree of hypertension, the moderate and severe patients occupied most. Among the complications of hypertension, heart failure, coronary heart disease, hyperuricemia and tachyarrhythmias in 2008 increased more than those in 1999 (p<0.01). The number of extremely high-risk groups in 2008 was apparently more than those in 1999 (P<0.01).2. In 1999 and 2008, the average blood pressure was (147.32±22.22)/(85.56±13.04) mmHg and (131.24±18.32)/(77.36±10.74) mmHg respectively, and attainment rate of blood pressure (<140/90mmHg) was 34.1%and 62.1%. It showed that the attainment rate of blood pressure was better in 2008 than that in 1999(p<0.01). The rate of the use of antihypertensive drugs was 88.2%in 1999 and 93.9%in 2008(p<0.01). In 1999, the top three anti-hypertension drugs were ACEI, CCB,βreceptor blockers, while, in 2008, they were ACEI, CCB, diuretics; The usage of the long-effected preparations was increasing. The rates of the drug combinations were 34.5%in 1999 and 79.2%in 2008 respectively. Apparently, the drug combinations increased much in 2008 than that in 1999 (P<0.01). In 1999 and 2008, the use of the two drug combinations accounted for 29.4%and 38.8%separately, and the chief ways of combination were CCB+ACEI(ARB), ACEI(ARB)+diuretics and ACEI (ARB)+p-blocker; the use of three or more drug combinations taked 14.1%and 40.9%separately, of which the major ways of combinations were CCB+diuretics+ACEI (ARB) and CCB+β-blocker+CEI (ARB)Conclusions:1. The average age of the in-patients with hypension and male patients in 2008 is higher than that in 1999. The patients combined with heart failure, coronary artery disease and hyperuricemia increased significantly, especially patients with extremely high risks.2.The attainment rate of blood pressure improved significantly while it was still not high.3. The use of antihypertensive drug conformed more to the guideline than that in 10 years before. While the guideline should be popularized better in basic hospital.
