| Objective:In order to understand the prevalence rate of osteoporotic fracture and the related factors of bone mineral density in the unique Hani ethnic minority, Yunnan. From October to November in 2008, we investigated a part of the Hani population and measured values of the right calcaneus quantitative ultrasound SOS and related observed measures in Green-Spring County, Hong-he County, Yuan-yang County of the Red River region, Yunnan. And collated the statistical data of medical examination in this surveyed, Analyzed and researched the prevalence rate of osteoporosic fractures and the related relationship, analysis the influence of their age, weight, body mass index, height and other indicators to bone mineral density. It could provided a reference about how to prevented and controlled the OPF of Hani people.Method:The modern statistical methods of many epidemiological investigation has been used, and full use of grass-roots network of health services, we had trained the local hospitals doctors and rural doctors. First there be given notice before the surveyed by the village committee, and then we excluded the residents of non-Hani, with the assistance of hospitals doctors, rural doctors and village head, By means of investigators to interview household survey and field inspection, Investigated the epidemiology of osteoporotic fracture in Hani ethnic minority; Measured and calculated the bone strength with the calcaneus Quantitative ultrasound bone densitometer, statistical analysis the correlation between Bone mineral density and age, height, weight, BMI and other relevance indicators in Hani ethnic minority. Before start to work, the investigating officers must be participated in intensive training, and unified approach, unified diagnostic criteria and processes would be applied. There was a senior orthopedic physician in Site investigation personnel who would be responsible for examining diagnosis and fill in the survey table whether was correct. The suspected patients of screening out would be sent to a local hospital for diagnosis by X-ray. The following was the content of researched and studied:①Interview survey:First of all, we interviewed and checked the Respondents and let they signed the informed consent, then they must completed the survey table about nutrition and health, which the content included the general information of respondents and the common risk factors for osteoporosis.②Dietary survey:Fill dietary questionnaire by food frequency method +24 hours recalled method. Food frequency:For more than 2 times per week ---often; average of less than 1 times per week---occasionally; an average of less than 1 times per 2 weeks---no. The pattern of in-kind proportion and 4 bowls which capacity was from small to large were used as a quantitative tool, assisted the respondents recalled the amount of food eaten in the past one day, and aided recall used Amount of oil and seasonings in each dish cooking, by 4 different size measuring spoons.③Bone mineral density measurements:We tested the right calcaneus ultrasonic with their point of order operations by calcaneal quantitative ultrasound meter (QUS) of Achilles Express which produced by company GE-Lunar U.S., the main parameters included ultrasound velocity (SOS) and calcaneal bone mineral density T values which could reflected the Bone strength.④Laboratory tests:Blood and extraction of respondents DNA, amplification the PCR, and analysis the gene polymorphism of VDR and OPG. ⑤tatistical analysis:The measure data was statistical analyzed by SPSS11.5 statistical software. The factorial analysis; single factor, multi-factor Logistic regression analysis; multiple data compared with the variance test; two sets of data compared with the t test; P<0.05 as statistically has significant difference, P<0.01 as statistically Has a very significant difference.Results:1.The Hani population situation of this surveyFrom October 15 to November 15,2008. Research group had investigated the residents of Hani ethnic minority whom living in Lu-chun, Yuan-yang and Red-River County in the Red River region, Yunnan. There were 25,834 people had been surveyed, of which 13,513 were male and 12,321 were female. There were 2543 people whose age was 60 years or older, including 1144 male and 1399 female. At the same time, we sampling over 50 years old Hani 300 people who living in Lu-chun, Red-River, Yuan-Yang three counties (109 males,36%; 191 women, accounting for 64%),They had been measured the calcaneus bone mineral density and then analyzed influence factors of bone mineral density, the minimum age was 50 years, the maximum age was 99 years, mean age was 71.25 years.2. The incidence of OPF in Hani ethnic minority in Red River region, Yunnan.Spine fractures were found in 35 cases (18 cases humpback including 16 females and 2 males,17 cases of spinal fractures which 14 females and 3 males), wrist fracture in 4 cases (including 3 cases of the right wrist fracture, left wrist fracture only 1 cases) who are women,2 cases of hip fracture were male, right knee fracture (patella) only 1 male. These patients were over 60 years old. Therefore, in this survey,there were 42 cases of Hani ethnic OPF in the Red River area, the prevalence was 1.63‰(42/25834), in which the male prevalence rate was 0.59‰(8/13513), female prevalence rate was 2.76‰(34/12321); The number of spine OPF was 35 cases, the prevalence was 1.35‰(35/25834), in which the male prevalence rate was 0.37‰(5/13513), female prevalence rate was 2.43‰(30/12321); The number of wrist OPF was 4 cases, the total prevalence rate was 0.15‰(4/25834), in which the male prevalence rate was 0 (0/13513), female prevalence rate was 0.32‰(4/ 12321); The knee OPF patient only one case, the prevalence was 0.04‰(1/25834), in which the male prevalence rate was 0.07‰(1/13513), female prevalence rate was 0 (0/12321); The hip OPF patients was 2 cases, the prevalence was 0.08‰(2/ 25834), in which the male prevalence rate was 0.15‰(2/13513), female prevalence rate was 0(0/12321).60 years and more older the Hani ethnic minority crowd in Red River region,Yunnan:The OPF prevalence rate was 16.52‰(42/2543), in which people over 60 years male prevalence rate was 6.99‰(8/1144) and female prevalence rate was 24.3‰(34/1399); The prevalence rate of spinal OPF was 13.76‰(35/2543), in which male prevalence rate was 4.37‰(5/1144), female prevalence rate was 21.44‰(30/1399); wrist OPF prevalence rate was 1.57‰(4/2543), in which people over 60 years the prevalence of males 0 (0/1144) and female prevalence rate was 2.86‰(4/1399); the knee OPF prevalence rate was 0.39‰(1/2543) in 60 years and older people, in which male prevalence rate was 0.87‰(1/1144) and female prevalence rate was 0 (0/1399); hip OPF2 cases, the prevalence was 0.79‰(2/2543), in which male prevalence rate was 1.75‰(2/1144) and female prevalence rate was 0 (0/1399) in 60 years and more older the Hani ethnic minority crowd.3. The calcaneus SOS value and other observed indicators in Hani ethnic minority in Red River region, Yunnan.The observed indicators included the number, age, height, waist circumference, body weight, body mass index, the right calcaneal quantitative ultrasound SOS value, the value of the right calcaneus bone mineral density T value and so on contents in this survey. The lightest weight was 30 Kg and the heaviest weight was 72Kg, the average weight was 46.39 Kg, body mass index as low as 14.27kg/m2, the maximum value was 36.13kg/m2, an average value was 21.07kg/m2,, the right calcaneal quantitative ultrasound SOS was 1512.38±36.77 m/s, In addition, there was the number of people's age, height, waist circumference, et al,and we had statistical analyzed the results of treatment used by SPSS version 11.5 statistical software. Mainly for the mean, variance, correlation and other analysis, p<0.05 was statistically significant:The correlation between the right calcaneus bone mineral density T values and the right calcaneal quantitative ultrasound SOS values (r= 0.890, P= 0) is high, so, it can predict the calcaneus bone density with SOS; The calcaneal quantitative ultrasound SOS value and body weight (r= 0.238, P= 0), body mass index (r= 0.132, P= 0.022) were positively related, but with age was negatively correlated (r=-0.524, P= 0), and all results were statistically significant.In addition, we can known from the results that the correlation between age and body weight (r=-0.319, P= 0), age and body mass index (r=-0.202, P= 0.001), age and calcaneal quantitative ultrasound SOS (r=-0.524, P= 0), age and bone mineral density T values (r=-0.535, P= 0) were negatively correlated, and P<0.01, there was statistically significant. In addition to the calcaneus quantitative ultrasound SOS and age was negatively correlated (r=-0.524, P= 0), The correlation between the calcaneus quantitative ultrasound SOS and body weight (r= 0.238, P= 0), body mass index (r= 0.132, P= 0.022), and the calcaneus BMD T values (r= 0.890, P= 0) were positively correlated, and were statistically significant. We also found that there was a significant difference between Hani calcaneal quantitative ultrasound SOS values and calcaneus bone mineral density T values in different gender survey, that men more than women. After the Hani stratified by age control study and found that:Hani men in the 50-59 group and 60-69 age group whose calcaneal quantitative ultrasound SOS values and calcaneus bone mineral density T value is no difference, and there was a significant group difference in 70-79 group. There are distinctive between the 50-59 age group and 60-69 years-old group in Hani women.and the differences would be more evident after 70 years old.Conclusion:1.The all prevalence of Hani people OPF was 1.63‰, in which male prevalence rate was 0.59‰, female was 2.76‰, and females than males in Red River region of Yunnan. In 60 years and older Hani people,the prevalence of OPF was 16.52‰, in which male prevalence rate was 6.99‰, female was 24.3‰, and females than males. 2.The female of Hani people were significantly lower bone mineral density after the age of 60, while men were significantly lower after 70 years in Red River region,Yunnan. In 50-69 year-old Hani:The SOS value of calcaneus bone mineral density was 1517.27±33.26 m/s in men, female 1504.96±39.63 m/s;The T value of the calcaneus bone mineral density was-0.82±1.11 in male and female average-1.2±1.28, there were no differences between men and women. More than 70 years: The SOS values of calcaneus BMD was 1523.56±32.55 m/s in men, for women was 1508.9±37.2 m/s; The T value of the calcaneus bone mineral density was-0.64±1.05 in male, and female was-1.06±0.94, there were differences between male and female and men than women.3.The correlation between the SOS values of calcaneal quantitative bone mineral density and the T value was well(r= 0.890, P= 0), and the SOS values can predict the bone mineral density in Hani people of Red River region,Yunnan. The SOS of calcaneal quantitative ultrasound with age was negative correlation, and the SOS values with body weight, body mass index, and the calcaneus bone mineral density T values were positively correlated. |
