Objective:By researching on the mammotome vacuum biopsy, the merit and demerit of this procedure was further analyzed to standardize the operation and its technique. Post-operative complications were also investigated, especially hemorrhage after the surgery, to clarify the causes of the complications and the preventive measures which should be taken.Methods:180 cases admitted with benign breast lesion to the 1st Affiliated Hospital of Jinan University were reviewed. The patients were divided into the mammotome group and the conventional surgery group (control group). Hemorrhage during the surgery was recorded, and factors associated with hemorrhage were analyzed including size, location and number of the mass, number of tunnels created. The patients were followed post surgery in terms of pain, fever, redness and swelling, brusing, hematoma, residual lesion and patient-perceived satisfaction. All the data were compared and analyzed for the factors which were responsible for hemorrhage.Results:The mammotome group was with shorter operation time, shorter time of wound healing and less amount of hemorrage by comparing with the conventional surgery group (P<0.05). The mammotome group had better cosmetic results. Incidence of complications, e.g. pain, fever, redness and swelling, was significantly lower in the mammotome group (P<0.05). Patients were significantly more satisfied with the surgical outcome of mammotome (P<0.05). By analyzing factors responsible for hemorrhage during surgery, excision of multiple lesions and mass in a larger size would significantly increase the rate of hemorrhage (P<0.05). Number of tunnel created during the mammotome surgery would not be affecting the rate of hemorrhage (P>0.05). By comparing the lesion location, excision of mass in the upper-outer quardrant presented with highest rate of hemorrhage (P<0.05).Conclusion:Mammotome vacuum biopsy would be able to effectively shorten the time of operation and the time of wound healing. This approach was less traumatized, with less hemorrhage, and the cosmetic result could be ensured. By fulfilling adequate training of minimally invasive surgery techniques, Mammotome could better reduce hemorrhage during the surgery and prevent the incidence of complication. The physical and mental trauma happened to the patient was greatly minmized and hence promoting patient-perceived satisfaction.
