Acne is a skin disease referring to the chronic inflammation of pilosebaceous glands. It belongs to "mian pao", "fen ci", "jiu ci", "fen pi", " fei feng fen ci" in TCM, and often occurs in face, protothorax and back, causing the severe skin damage such as comedo, pimpliness, pustule, scobination, cystis. The ancient doctors of TCM recognized that acne resulting from the external-pathology as wind, heat, cold, damp and pathological condition as Lung-heat, Blood-heat, asthenia. In recent years, many doctors of TCM have new understanding on the cause of acne. They consider that Asthenia of the Liver and Kidney, Blood stasis, Phlegm obstruction, the incoordination between vital meridian and conception vessel, etc., as importance pathogenesis for acne. Meanwhile,they get satisfactory curative effect in clinic. The origin of acne in disease process is not fully understood in Western medicine. However, there are indications that it frequently occurs in association with dyssecretosis of gonadal hormone, dyskeratosis of pilosebaceous glands, microbe in follicle, inflammatory damage, immunoreaction, psychologic factors, inheritance, etc. With the quickening pace of modern life, the progressively life stress, and the population of unhealthy routines,the incidence rate and recurernce rate of Acne are rising gradually. Acne, as a skin disease disfiguring patients, also severely influences their psychological state and quality of life. In clinic, a number of researches and therapies on Acne mostly focus on the standpoint of biomedical model. With the change of medical modality, the importance of patients'psychological state and quality of life is gaining recognition. And both of them become research hotspot.On the base of applying Chinese medicinals and acupuncture according to syndrome differentiation and considering patients'psychological state, we apply the Global Acne System (GAGS),Self-Depression Scale (SDS), Chinese quality of life (CQOL) and recurernce rate of acne to observed the effect of Si hua points blood pricking therapy with emotional therapy of TCM in improving patients'clinical symptoms and their psychological state and quality of life. Through the clinical research, we aim to discuss the mechanism of Si hua points blood pricking therapy with emotional therapy of TCM therapy, and explore effective and new methods for patients'physical and psychological health.In this study,69 cases qualified to our research were divided into treatment group and control group randomly. According to syndrome differentiation,the control groups accepted the Chinese medicinals decocted in water for oral dose and acupuncture treatment. Meanwhile, the pusembolus in skin lesions should be removed by needling instrument disinfected by high compressed steam. On the basis treatment of which, the patients in the treatment group were treated by Si hua points blood pricking. Firstly, we give massage around the acupoints to make the local skin congested.After disinfecting the partial skin strictly, we use the triangle-edged needle to puncture si hua points quickly. Each acupoint needs 3 or 4 points. Then cupping them for 5-10 minutes to make 3-5ml bleeding volume. Whereafter, we puncture the other points by the disposable needle of Hua Tuo Brand with a required size of 0.30×30mm or 0.30×25mm and take the reinforcing and reducing manipulation by raising or inserting and twisting or transferring needle. Among which, we use reinforcing method for the deficiency, and vice versa. During the couse of acupuncture therapy, We combine with emotional therapy of TCM. Treatment course:2 times per week,8 times as one course. After one treatment course, we observed the improvement of the lesional skin condition trough the GAGS, and evalua the level of SDS and CQOL. Besides that, in order to observe the recurernce rate of Acne, all patients were followed up during the treatment and after 3 months. Finally, the results were analyzed by statistics.After one treatment course, both of two groups have great improvement in the lesional skin condition and the level of SDS and CQOL. Contrasting the curative effect between two groups, the cure rates of the treatment group was 22.84%, and the total efficiency rate was 94.29%. In the control group, the cure rates was 11.76%, and the total efficiency rate was 82.35%. Contrasting the effect between two groups by statistical analysis, They have notable difference (P<0.05). It indicates that the improvement of lesional skin condition in the treatment group is better than the control group. Moreover, comparing the level of SDS and CQOL of two groups, it indicates that treatment group is superior to the contrl group. They have notable difference (P<0.05).It further shows that improvement of the degree of patients'depression state and the life quality of patients are better than the control group. Besides, all patients were followed up after 3 months. the recurernce rate of the treatment group was 5.71%, the control group was 52.94%. They have notable difference (P< 0.05). It indicates that the treatment group has good long term effectiveness.In the study, through the clinical research of the treatment of Si hua points blood pricking therapy with emotional therapy of TCM for acne, we concluded that:1. Both the treatment of Si hua points blood pricking therapy with emotional therapy of TCM and the Chinese medicinals therapy is effective on acne. while the total efficiency rate of the treatment group is better than the control group.2. The treatment of Si hua points blood pricking therapy with emotional therapy of TCM improved patients'lesional skin condition, and the level of SDS and CQOL obviously.3. Under the guidance of the new modern medical model of biopsychosocial, we consider the patients'psychological state as an important pathogenetic factor. And Si hua points blood pricking therapy with emotional therapy are effective and humanized in the treatment for acne, and worth applicating and popularizing in clinic.
