ObjectiveOur purpose is to investigate various aspects of the elderly hypertensive patients'proper diet, exercise, smoking, drinking, mental state, hypertension knowledge, and drug compliance. We will investigate that the behavior of factors affect on blood pressure control and degree of effect in elderly hypertensive patients. Our study is aimed to provide theoretical basis and practice guidance for Community prevention and control of hypertension. At the same, investigate the Causes and mechanisms what were the behavioral factors influence the treatment effect.MethodsThis study used cross-sectional survey methodology, and select an area to investigate the site in Tianjin.229 aged hypertensive patients of a health center in Tianjin, who received NAHC treatment, were investigated. The height, weight, waist circumference, hip circumference, heart rate were measured once, blood pressure of right upper arm of patients seated twice on the spot; A questionnaire survey was also conducted over their background information including age, sex, civil state, living conditions, education, medical history, family history and their Behavioral factors, such as: smoking, alcohol consumption, exercise, sleep, mood, character and drug compliance. Their information of blood pressure, height, weight, waistline, hip circumference before treatment was collected from historical file data.In this study, alterations in systolic and diastolic blood pressure value of chosen patients after the treatment in comparison with the value before the treatment reflect the effect of the treatment on their blood pressure control. Decreased during the said values. Samples t test and chi-square test were used to analyze the effect of changes in blood pressure control. We have described the status of patients with behavioral factors.The multiple linear regression model was used to analyze the effect of different factors on change value of systolic and diastolic pressure of elderly hypertensive patients with community prevention and control.Results229 hypertensive patients were investigated who have taken the same hypertension drugs and community management plan. The number of first stage hypertensive patients were 132 (57.6%), the number of second stage were 80 (34.9%), the number of third stage were 17 (7.4%), respectively. The number of hypertensive patients whose blood pressure were under 140/90 mmHg is 53(23.1%) and 85(37.5%) pre and post management. The rate of blood pressure control were significantly improved (P<0.05).Average value of systolic and diastolic blood pressure of research object were 150.3 mmHg/87.5 mmHg and 144.2 mmHg/83.5 mmHg respectively pre and post management. The blood pressure decrease value of first stage hypertensive patients, second stage hypertensive patients, third stage hypertensive patients were 3.88 mmHg,3.66 mmHg, 6.32 mmHg,4.47 mmHg ,22.24 mmHg,3.98 mmHg pre and post management. The blood pressure of hypertensive patients decreased significantly pre and post management.The rate of smoking, drinking, dietary equilibrium, physical exercising, over standard of BMI, obesity, waist hip ratio above 0.9, hypertension knowledge(6 points and above), living alone, divorce or spouse, drug compliance(11 points above) in the investigation objects were 21.4%, 24.5%, 41.9%, 62.5%, 43.2%, 37.6%, 47.2%, 85.2%, 13.5%, 24%, 81.2%, respectively.The main influence factors of first stage hypertensive patients were hypertension knowledge, body mass index(BMI), living conditions, marital status and drug compliance with blood pressure controlled pre-management. With the method of backward stepwise regression screening variable, the influence factors of systolic blood were respectively hypertension knowledge, body mass index(BMI) and living conditions, according to contribution rate from big to small。The contribution rate of hypertension knowledge, body mass index(BMI) and living conditions were respectively 0.16, -0.15, 0.14;the influence factors of diastolic blood were respectively marital status, drug compliance and hypertension knowledge, according to contribution rate from big to small。The contribution rate of marital status, drug compliance and hypertension knowledge were respectively -0.17, 0.15, 0.11。The main influence factors of second stage hypertensive patients were hypertension knowledge, education level and weekly alcohol intake with blood pressure controlled pre-management. With the method of backward stepwise regression screening variable, the influence factors of systolic blood were respectively hypertension knowledge and education level, according to contribution rate from big to small。The contribution rate of hypertension knowledge and education level were respectively 0.25,-0.15;the influence factors of diastolic blood was respectively weekly alcohol intake. The contribution rate of weekly alcohol intake was -0.16。ConclusionThere are many behavioral factors that they affect the treatment of hypertension, Such as blood pressure levels of before management,drug compliance, hypertension knowledge, body mass index (BMI), living condition, education level, Marital status,alcohol consumption. the main behavioral factors that it affect the control effect of SBP is decreasing order: pressure levels of before management, hypertension knowledge, BMI, living condition, education level; the main behavioral factors that it affect the control effect of DBP is decreasing order: pressure levels of before management, Marital status, drug compliance, alcohol consumption.
