Objective:To investigate the alteration between arterial stiffness and the hypertensive patients with different risk stratification,and analyze the factors contributed to.Methods:1632 hypertensive patients were selected and divided into low risk (97), moderate risk (678), high risk (599) and very high risk (258) four groups and 1758 normal blood pressure people as control group from whom had been routine physical examination at the third Xiangya hospital from January 2008 to January 2009.Brachial ankle's pulse wave velocity (BaPWV) and ankle-brachial index (ABI) were measured of all the people by using Omron made of Japan for detection of atherosclerosis, comparing the differences of arterial stiffness of the patients in different risk stratification groups, and analyzing the factors contributed to.Results:1. The comparison between different risk stratifications of hypertension:BaPWV of people in normal blood pressure group, low risk group, moderate risk group, high risk group and very high risk group were (1264±156) cm/s, (1507±200) cm/s, (1615±307) cm/s, (1672±314) cm/s and (1890±428) cm/s respectively. The differences among the five groups was significant (P<0.05). The BaPWV of patients in hypertension risk stratification in each group compared to that of the normal blood pressure group were significantly higher (P<0.05), BaPWV of patients was compared to different risk stratifications between each group were significantly different (P<0.05). ABI of people in normal blood pressure group, low risk group, moderate risk group, high risk group and very high risk group were (1.10±0.07), (1.10±0.07), (1.13±0.07), (1.14±0.07) and (1.14±0.09), the differences among the five groups were significant (P<0.05). ABI of patients in moderate risk group, high risk group and very high risk group were higher than those in low risk group and normal blood pressure group, the differences were significant (P<0.05). ABI among the patients of moderate risk, high risk and very high risk group were no significant difference and also no difference between the patients of low risk group and normal blood pressure group (P>0.05).2. The age, body mass index (BMI), fasting blood glucose (FBs), the systolic pressure (SBP) and Creatinine (CREA) were the influential factors of BaPWV (P<0.05). And the age, gender, body mass index (BMI), the systolic pressure and Creatinine (CREA) were the influential factors of ABI (P<0.05).Conclusion:1. BaPWV could be used as an evaluation index for the risk stratification of hypertensive patients.2. The BaPWV in different risk classifications of hypertensive patients were mainly correlated with age and systolic pressure.
