| Objective1. To research the optimal catheterization path, the normal angiography anatomy and the common variation of rabbit's hepatic vessels.2. First study the advantages and limitations of hepatic arterial infusion under temporary hepatic circulation occlusion.Methods1.18 rabbits were randomly choose, the right femoral artery and vein of 6 rabbits were visualized punctured by improved Selding's method, and were inserted sheath, Coude's catheter and coaxial-micro catheter. Angiographies of celiac artery, common hepatic artery, proper hepatic artery, inferior vena cava and hepatic veins were performed on all the rabbits in turn. The position, branching and direction of these vessels were studied in the angiography imaging and by surgical dissection.2.12 rabbits were randomly divided into two groups:hepatic artery infusion group (HAI group) and hepatic artery infusion under temporary hepatic circulation occlusion group (HAI-THCO group). The right femoral artery and vein of all rabbits were visualized punctured by improved Selding's method. The micro catheters were separately inserted into proper hepatic artery and right hepatic vein. For HAI group, 5-FU (10mg/ml) was infused into common hepatic artery by high pressure injector according to rabbits'body weight (100 mg/kg) for 10 minutes. For HAI-THCO group, 5-FU was infused as HAI group for 10 minutes, but the common hepatic artery and hepatic portal vein were temporary occlusion by artery clamp when 5-FU was being infused, for 15 minutes. Begin infusion, for two groups, at the 2 min,5 min,10 min, 15 min,20 min and 30 min, the hepatic circulation blood sample was collected from right hepatic vein, and systemic circulation blood sample from inferior vena cava,1 ml per time. After ultracentrifugation, the blood drug level of these blood samples was determined by high performance liquid chromatography (HPLC).Relults1. The catheterization and angiography of hepatic vessels were successful. The mean time was about 30 minutes and the mean blood loss was less than 2 ml.2.The origin location of rabbit's celiac artery was commonly between the inferior 1/3 of the 12th thoracic vertebrae and the superior 1/3 of the 1st lumbar vertebrae.The branching and direction of the rabbit's celiac artery and hepatic artery was different from human.3.For HAI group, apart from 20 min and 30 min after infusion, the blood drug level of liver circulation was higher than that of systemic circulation(p<0.05).But for HAI-THCO group, the blood drug level of hepatic circulation was higher than that of systemic circulation at all time (p<0.05).The hepatic circulation blood drug level of HAI-THCO group was always higher than that of HAI group (p<0.05),but the systemic circulation blood drug level of HAI-THCO group was always lower (p<0.05).4. The hepatic circulation maximum concentration (Cmax) of blood drug of HAI-THCO group and HAI group was (23.057±3.270) (μg/ml) and (4.408±1.092) (μg/ml) respectively, the Cmax of HAI-THCO group was higher (p<0.05), it was 5.23 times of that of HAI group. The systemic circulation Cmax of two groups was (1.456±0.217) (μg/ml) and (2.335±0.669) (μg/ml) respectively, the Cmax of HAI group was higer (p<0.05), it was 1.60 times of that of HAI-THCO group. The hepatic circulation AUC of HAI-THCO group and HAI group was (368.927±52.416)μg/(ml-min) and (65.630±14.928)μg/(ml-min) respectively, the AUC of HAI-THCO group was higher(p<0.05),it was 5.62 times of that of HAI group. The systemic circulation AUC of two groups was (27.193±3.948)μg/(ml-min) and (45.301±12.275)μg/(ml·min) respectively, the AUC of HAI group was higer (p<0.05), it was 1.67 times of that of HAI-THCO group.Conclusion1. The catheterization of rabbit's hepatic artery from femoral artery under fluoroscopy was a method of simple, light injury and high achievement ratio.2. The normal angiography anatomy and the common variation of rabbit's hepatic artery should be studied, and it was helpful to the interventional therapy research through rabbit's hepatic artery. 3. HAI-THCO was a simple and effective hepatic location infusion chemotherapy technique, and it had clinical feasibility.4. Compared with HAI, HAI-THCO not only could increase the blood drug level of hepatic circulation, but decrease the blood drug level of systemic circulation.5. HAI-THCO had some limitations; it needed to be further studied. |
