Objective: Aim to explore the relationship of cognitive impairment and the cerebral blood flow and hemodynamics in patients with ischemic stroke.Methods: Recruited inpatients who suffered from acute ischemic cerebrovascular disease and health people matched in age, sex, education, and vascular risk factors(diabetes, hypertension,hyperlipidemia, and smoking) as control subjects, who were recruiteded from Healthy check-up center. Middle cerebral artery and basilar artery were examined by means of transcranial Doppler instrument to obtain mean velocity, pulsatility Index and resistance Index in all subjects. We evaluated CVR of the bilateral middle cerebral arteries dy the breath-holding test and calculated the breath-holding index. Mini Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA) were used to evaluated patients'cognition.All ischemic stroke patients were underwent MRI and CT scan.Results: 1.The hemodynamics results of the ischemic stroke patients.1.1There was significant difference between the ischemic stroke group and control group in the proportion of Intracanial artery stenosis and extracranial artety stenosis.1.2 There was significant difference between the ischemic stroke group and control group in the pulsatility index and breath-holding index.2. There was significant difference between the Small-Vessel Disease group and large Vessel Disease group in the pulsatility index and breath-holding index.3. The results of cognitive functional test3.1 There was significant difference between the ischemic stroke group and the control group in the score of MMSE and MoCA.3.2 There was significant difference between the ischemic stroke group and the control group in the score of MMSE and MoCA abnomal .4. There was significant difference between the Small-Vessel Disease group and the large Vessel Disease group in the score of MMSE and MoCA.5. The relationship of cognitive impairment with the cerebral blood flow and hemodynamics in patients with ischemic stroke5.1 There was significant difference between the MMSE,MoCA abnomal group and the MMSE,MoCA nomal group in the pulsatility index and breath-holding index.5.2 There had not correlation between MMSE,MoCA and Vm; There had not correlation between MMSE,MoCA and RI;There had significant positive correlation between MMSE,MoCA and BHI; There had significant negative correlation between MMSE,MoCA and PI;Conclusion: 1The cerebral vasomotor reaetivity was significantly impaired in the Small-Vessel Disease patients;The Small-Vessel Disease patients are easy with post cognitive impairment after stroke.2 There had significant correlation between MMSE,MoCA and BHI; There had significant correlation between MMSE,MoCA and PI; There had significant correlation between cognitive impairment and the cerebral hemodynamic changes in ischemic stroke patients. |
