| Objective:The purpose of the present study is to assess the difference of humidification performance between oxygen dynamic atomization or HME. Thus we can choose the most suitable nursing method for patients.Methods: The neurosurgery ICU and SICU in the top three hospitals in Xinjiang province were chosen as a test base. The 60 cases of patients with traumatic brain injury in tracheotomy were chosen for the research .They divided randomly into test group 1 and test group 2. The experimental group 1 patients were pulverized by HME in following 4 days and the experimental group 2 patients were pulverized by oxygen dynamic atomization in following 4 days .Observing patients'respiratory (RR), heart rate (HR), systolic blood pressure (systolic pressure), diastolic blood pressure (diastolic pressure), PaCO2, PaO2, SaO2, daily sputum times, daily suction sputum volume and sputum characteristics (â…¢) and complications or not (airway mucous membrane bleeding, phlegm callus formation, the occurrence of pulmonary infection in comparison).Results: The difference was statistically significant. Breathing (RR), heart rates (HR) of Patients with HME were speeding up compared with oxygen dynamic atomization; diastolic blood pressure (diastolic pressure) increased. Therefore, effect of patients by oxygen dynamic atomization is better than by HME here; Comparison of sputum characteristics (â…¢phlegm) and PaCO2 showed no statistical difference (p>0.05); But for PaO2 and SaO2 compared (p<0.05). So oxygen dynamic atomization can maintain higher PaO2 and SaO2 than HME; There were statistical difference in suction sputum times and suction sputum volume compared (p<0.05). To compare with HME, patients with oxygen dynamic atomization increased daily volume of sputum and times of suction. The occurrence of pulmonary infection, mucosal bleeding and the incidence of sputum callus showed no significant difference (p>0.05).Conclusion: (1) Patients with oxygen dynamic atomization are more comfortable than with HME. (2) Patients with oxygen dynamic atomization can maintain higher blood oxygen partial pressure than with HME. Although PacO2 of Patients in the application of HME were slightly higher than in the oxygen spray, but there were not two kinds of wet-based methods of hypoventilation or hypoventilation phenomenon. (3) The occurrence ofâ…¢phlegm pulmonary infection, mucosal bleeding and the incidence of sputum callus show no significant difference. |
