Objective: To explore the value of serum anti-Mullerian measurement in predicting ovarian response in infertile patients undergoing controlled ovarian hyperstimulation (COH) for Assisted Reproductive Technology (ART). Methods: A total of 68 patients undergoing their cycles of IVF/ICSI treatment were studied. According to number of oocytes retrieved and peak E2 concentration, the 68 patients were divided into three groups: poor ovarian response group ( group A), normal response group ( group B),high response group ( group C). Serum estradiol (E2), progesterone(P),anti-Mullerian hormone (AMH), serum levels were detected on day 2 of menstruation(basal level) and HCG injection day and through the characteristics of the treatment cycle and AMH,E2 ,P levels were analyzed. According to the outcome of pregnant,the ET-patients were divided into two groups: pregnant group and nonpregnant group,and than comparing the differences between them. Results: (1)In the three groups, the data which means median basal serum AMH level and median serum AMH level on HCG injection day are all different(P<0.01). Median basal serum AMH levels in the group A, group B and group C were 0.645 ng/ml,1.762 ng/ml,3.101ng/ml respectively ;median serum AMH levels on HCG injection day in the group A, group B and group C were 0.331 ng/ml,0.898 ng/ml,1.434ng/ml respectively; through the multiple compare we can find the levels of the D2AMH and the HCGAMH have such a rule (group C>group B> group A). (2) It shows there is positive correlation between oocytes retrieved and basal AMH, AMH of HCG injection day, AFC and progesterone of HCG injection day, but the negative correlation between oocytes retrieved and basal FSH and the quantity of Gn. (3) There is positive correlation between OHSS and the quantity of Gn ,AFC,D2AMH,HCGE2 and HCGAMH, but the negative correlation between OHSS and oocytes retrieved and basal FSH . (4)There are differences between good embryo numbers in the pregnant group and nonpregant group. Conclusions:(1)basal serum AMH,serum AMH of HCG injection day and AFC are the better marks of ovarian response; (2) basal serum AMH,serum AMH of HCG injection day and E2 of HCG injection day may offer accurate prediction of the OHSS; (3) serum AMH can not offer prediction of the pregnancy outcome. |
