| research backgroundChronic atrophic gastritis(CAG) is a type of Chronic gastritis, its duration is long, the recurrence rate is higher, is the frequently-occurring disease of digestive system, refractory disease, secondly the CAG from normal gastric mucosa to inflammation, atrophy, intestinal, hyperplasia, the evolution law of gastric cancer has been generally accepted.The CAG has clear is classified as precancerous disease, it is a serious threat to human health. Asia is the main area of chronic atrophic gastritis, with China and Japan has the highest incidence of a disease, and there are quite a number of patients will finally deteriorate into the stomach. In the age distribution, chronic atrophic gastritis is a common disease and frequently encountered disease in the elderly, the incidence with age also increased accordingly. So, early detection and identification of precancerous lesions, for effective follow-up monitoring, best can intervention and block the deterioration become one of the effective measures to reduce morbidity and mortality.CAG, as a kind of chronic digestive system diseases caused by a variety of pathogenic factors, the current of modern medicine in the treatment of it is still no special and effective methods, or treatment is given priority to with symptomatic treatment.and western medicine treatment of recurrence rate is high, the side effects more disadvantages exist. And traditional Chinese medicine in the treatment of CAG, relying on reasonable treatment based on syndrome differentiation, and using the traditional Chinese medicine, proprietary Chinese medicine, acupuncture and other treatments, has obtained the remarkable curative effect. Treatment of traditional Chinese medicine not only can be effective in a short time to improve the patient’s clinical symptoms, the most important thing is that it could partly reverse pathological changes of gastric mucosa have occurred,thus further to prevent the occurrence of chronic atrophic gastritis canceration. So Chinese medicine in the treatment of CAG has huge advantages and broad prospects.Research objectivesThis study is on my tutor ZhouBin’s clinical experience in the treatment of CAG, By observing experience side "Wen Yang Jian Pi decoction" to treat the clinical curative effect of spleen and stomach weak type of CAG and through professional statistical analysis on the objective evaluation to provide TCM Dialectical treatment of spleen and stomach weak type chronic atrophic gastritis clinical thinking and the basis.Research contentsThrough long-term clinical practice,Dr.Zhou summarizes the spleen and stomach weakness is the main pathogenesis of CAG,And combined with traditional Chinese medicine on the spleen kidney "nature" and "nurture" theory of engenders relationships to each other as the basic therapeutic principle of "Wen Yang Jian Pian decoction" to treat spleen and stomach weak type CAG. The aim of the study is to observe its treatment of spleen and stomach weak clinical curative effect of CAG.This study is divided into a total of literature review and clinical study of two parts.Literature review:We systematically reviewed and summarized the diagnosis and treatment of CAG in modern medicine development.we also expound in detail the scope of TCM of CAG,type of TCM etiology and pathogenesis, syndrome differentiation and syndrome differentiation treatment.Clinical trials research:we collect TCM syndrome of spleen and stomach weak cases of patients with CAG, and applicate "Wen Yang Jian Pian decoction" to treat patients for 12 weeks.by comparing the patients clinical symptoms and the results of gastroscope changes before and after medication, we explore the treatment of "Wen Yang Ji Pian decoction" for spleen and stomach weak clinical curative effect of chronic atrophic gastritis.Research resultsThe results showed that "Wen Yang Jian Pian decoction" in treating spleen and stomach weak type of CAG has a significant curative effect, the treatment the total effective rate was 90.59%, the recovery rate is 10.59%, the efficiency of 52.94%, effective rate was 27.06%. In terms of pathological curative effect, gastric mucosa atrophy in gastric antrum of the total effective rate was 83.05%, the gastric body was 100%, the Angle of the stomach is 64.52%; Intestinal metaplasia in gastric antrum of the total effective rate was 90.76%, and the gastric body is 73.33%, the stomach Angle is 73.33%; This study hyperplasia in the gastric antrum and the gastric body and gastric Angle does not appear invalid cases.Conclusions"Wen Yang Jian Pian decoction" in treating spleen and stomach weak type of CAG has a good effect.It can not only improve the patient’s clinical symptoms, also can reverse atrophy, intestinal metaplasia and hyperplasiain of CAG in the different degree. And as the extension of treatment time, the patient’s clinical symptoms and the improvement of the gastric mucosa pathological degree is increasing. In addition, through a period of treatment after the follow-up,we found that stopped taking "Wen Yang Jian Pian decoction", patients can still get continue to ease some of the symptoms.Therefore, in the treatment of CAG, Chinese medicine has a distinct and lasting effect, And it is able to reduce the recurrent cases after drug withdrawal symptoms. |
