| Research Background:Chronic atrophic gastritis (CAG) is a common disease of digestive system, which had the characters of shringking of the gastric mucosa, decreaseing or disappearing of the gastric glandular and often evolved from chronic superficial gastritis. The disease with long course, tending to recrudesce is very difficult to be cured. The rate which chronic atrophic gastritis developed into cancer was about2%-8%, which has high morbidity and mortality seriously endangers human health. Most patients of chronic atrophic gastritis were elder persons, and along with growth of the age the morbidity was also subsequently heighten. One way to lower the morbidity and mortality of stomach cancer is to early identify premalignant lesion then effectively monitor and intervene.At present, western medicine think shringking of the gastric mucosa difficult to reverse, whose treatment of CAG ways and means can do a check against treatment, regular review and lack of an ideal means and methods. Compared with Western medicine, Chinese medicine has its unique advantages in the diagnosis and treatment. The TCM can effectively reverse atrophic gastric mucosa, intestinal metaplasia and atypical hyperplasia by diagnosis and treatment. This has the important meaning for preventing and controlling precancerous lesions.Research objectives:On the basis of the exploiting and coordinating the clinical experience of my tutor Zhou Bin on CAG, we observe and evaluate the clinical therapeutic effect of treating chronic atrophic gastritis of spleen-stomach deficiency syndrome by Jian Pi Xiaopi decoction for the treatmeng of the CAG to provide ideas and reference.Research Contents:Dr.Zhou considers weakness of the spleen and stomach is the main pathogenesis of CAG.He treated the spleen-stomach deficiency type CAG by JianPi Xiaopi decoction which is composed by the drugs of warming and tonifying spleen and stomach. The aim of the study is to observe the clinical therapeutic effect of Jian Pi Xiao pi tang on patients with CAG of pleen-stomach deficiency type.There are two major parts of this thesis:document review and clinical experimental study.Document review:First, we systematically review the modern medical research of the CAG. Second we summarize the scope of CAG in TCM, the disease cause, pathogenesis,the syndrome differentiation classification, and the treatment.Empirical study:After understanding of category of CAG, its clinical manifestation, differential classification, etiology and pathogenesis, dialectical therapy in TCM, the study will aim to observe the clinical therapeutic effect of treating chronic atrophic gastritis of spleen-stomach deficiency syndrome by Jian Pi Xiaopi decoction through the comparison of symptoms and gastric mucosa pathology before and after the treatment.Research Results:Clinical observation shows Jian Pi Xiaopi decoction to have distinctly effective to cure chronic atrophic gastritis of spleen-stomach deficiency syndrome. The clinical symptom were imporoved and the total curativr effect was91.43%, which8.57%of the cases were cured and obvious curative rate and effective ratio are25.7%and57.1%respectively. Gastric mucosa pathology were imporoved and the total curative effect was83.33%, which43.33%of the cases were cured and obvious curative rate and effective ratio are23.33%and16.67%respectively.Conclusion:1.There are some factors could raise the incidence rate of the spleen-stomach deficiency type CAG, for example, the family history of gastric cancer, Hp infection, eat and drink too much, say up too late, insomnia, fatigues, depression, mental tension, and so on.There are more women than men in these patients, most of whom are over55years old.2. Jian Pi Xiao pi decoctionis is quite effective in treating spleen-stomach deficiency type CAG. The treatment effect is proportional to the course of treatment, the longer the treatment lasted, the better the curative effect. |
