| Objective:1.To explore the risk factors of Alzheimer’s disease, vascular dementia and mixed dementia, and to provide scientific evidence for the prevention of dementia.2.To explore the correlation between TCM syndrome and cognitive function in patients with mild cognitive impairment, to provide scientific basis for TCM syndrome differentiation and treatment.Methods:A total of 2183 patients’ data in 3rd department were selected, including basic information survey and neuropsychological assessment scale.Through comprehensive evaluation, screening out Alzheimer’s disease 407 cases, vascular dementia 163 cases, mixed dementia 135 cases and normal control 1478 cases. The TCM Syndromes of 20 patients with Mild Cognitive Impairment were judged.Using SPSS17.0 logistic regression and multiple linear regression to screen out related risk factors, using spearman correlation to analysis the relationship between TCM syndrome of aMCI patients and cognitive function, P< 0.05 for differences were statistically significant.Results:l.The average age of AD group, VaD group and MD group were higher than that of NC group (P=0.000). The average education years of AD group, VaD group and MD group were lower than that of NC group (P=0.000). Age had positive relationship with AD, VaD, MD and dementia,P values were 0.000,0.001,0.000,0.000. Age had negative relationship with MMSE scores,P values were O.OOO.Education had negative relationship with AD, VaD, MD and dementia,P values were 0.000,0.001,0.000,0.000. Education had positive relationship with MMSE scores,P values were 0.000.There were significant differences in gender of all groups (P=0.000), but there was no significant correlation with the disease infaction.2.BMI index between AD group and NC group was significantly different(P=0.000), the same result as comparing VaD group to NC group(P=0.000),and MD group and AD group(P=0.000).Both the lower BMI index group and the normal BMI index group had positive correlation with MD group, AD group and dementia group.The P values of AD group were 0.002 and 0.008,while in MD group were 0.003 and 0.004.The dementia group showed more significantly P values as 0.000 and 0.001. Different level BMI index had different correlation with MMSE, which was negatively correlated with the lower-weight(P=0.037),and was positively correlated with overweight and obesity. The P values were 0.033 and 0.000.3.Mental labor had positive correlation with VaD,AD and dementia, the P values were 0.008,0.003 and 0.001. However, due to the lack of professional items (53.8%), the correlation between mental labor and the disease needs to be further studied.4.Alcohol consumption had positive correlation with AD and dementia,P values were 0.002 and 0.001, and the OR values were 3.133 and 2.98.Whether drinking now was negatively correlated with each group, P values were 0.000,0.005,0.001 and 0.000, OR values were 0.164,0.219,0.142,0.175.5. Whether living independently had negative correlation between all groups of dementia, and positive correlation with MMSE, all those P values were 0.000.6.Having hobbies had negative correlation with AD,VaD and dementia,P values were 0.041,0.004 and 0.005, and positive correlation with MMSE, P value was 0.003.7.High blood pressure, cerebral infarction, cerebral hemorrhage had positive correlation with VaD,P values were 0.003,0.000 and 0.000. Cerebral hemorrhage had positive correlation with dementia, P value was 0.027.8.Introverted personality had negative correlation with MMSE,P value was 0.003. Depression had positive correlation with AD, P value was 0.045.Widowhood had positive correlation with AD and dementia, P values were 0.03 and 0.017.9.The syndrome elements of patients with aMCI appeared as followed:kidney deficiency (50%), Yang deficiency (50%), medulla deficiency(50%), phlegm (50%), Qi deficiency(40%),those above were high frequent,and blood deficiency (5%), heat (0%), blood stasis (5%) shared the lower frequency.10.Patients with aMCI in Qi deficiency type had negative correlation with ADL, P value was 0.045.Yang deficiency type had negative correlation with word recognition, P value was 0.005.Sputum turbidity type had negative correlation with MMSE,P value was 0.015.Education had positive correlation with MMSE,P value was 0.003.BMI index had positive correlation with the logical story Ⅱ,P value was 0.032.Conclusions:1. Aging and lower level of education were the risk factors of dementia.People who had stronger independent living ability were more likely to share the lower risk of dementia.=2. Alcohol consumption,depression, widowhood and lack of hobbies would increase the risk of AD.Introverted personality traits may be more prone to cognitive decline.3.The BMI index of dementia patients may be lower than the normal value, and the early warning effect for dementia need to be further studied.4.Cerebrovascular events,lack of hobbies may be risk factors for vascular dementia.5.Phlegm, Qi deficiency and yang deficiency may be related to the cognitive function of aMCI patients, and the cognitive function may decrease with the different degrees of these syndromes. |
