Study On The Correlation Between The Characteristics Of TCM Constitution Distribution And The Characteristics Of Cognitive Impairment And Its Risk Factors In Patients With Mild Cognitive Impairment

Objective:To explore the correlation between TCM constitution distribution characteristics,cognitive impairment characteristics and risk factors in patients with mild cognitive impairment,in order to further clarify the relationship among them,find the breakthrough point for prevention and treatment of this disease,and provide more targeted basis for prevention of mild cognitive impairment before disease and prevention of disease.Method: According to the inclusion and exclusion criteria,the research objects were determined,and the constitution types of the included cases were determined by using the Constitution Judgment Table of Traditional Chinese Medicine.At the same time,the general situation and risk factors of the included cases were collected and statistically analyzed by SPSS26.0.Results:1、In the distribution of TCM constitution types of the cases included in this study,qi deficiency(33.6%),yang deficiency(21.5%)and yin deficiency(15.9%)are the most distributed,while peace quality(1.9%),special quality(1.9%)and blood stasis quality(1.9%)are the least distributed;2、Chi-square test showed that there were significant differences in BMI,smoking history and diabetes among MCI constitution groups,while univariate analysis of variance and chi-square test showed that there were no significant differences in age,sex,education level,drinking history,hyperlipidemia and hypertension among the groups;3、By one-way ANOVA,the total score of Mo CA has statistical significance among MCI constitution types(P=0.043),among which the total scores of Yang deficiency and Yin deficiency are the lowest.Further multiple comparisons are made on the total scores of Mo CA of each constitution type.There are statistical differences between Yang deficiency constitution and Yin deficiency constitution and Qi deficiency constitution respectively,and there is no statistical difference in the total scores of Mo CA between other constitution types.According to rank sum test,there are statistical differences in visual space and executive function scores among different constitution types(P=0.000),among which the score of yang deficiency is the lowest.After pairwise comparison,there are differences in visual space and executive function scores between yang deficiency constitution and qi deficiency constitution,yin deficiency constitution and damp heat constitution respectively,but there is no significant difference between other constitution types.After rank sum test,there are statistical differences in language ability scores among different constitution types(P=0.000),and the scores of damp-heat quality are the lowest.Further multiple comparisons show that there are differences in language ability scores between damp heat constitution and Qi-deficiency constitution and Yang-deficiency constitution respectively,and there is no difference between them in other constitution types.According to rank sum test,the scores of delayed recall have statistical differences among different constitution types(P=0.003),and the scores of yin deficiency and yang deficiency are the lowest.In further pairwise comparison,there are statistical differences between yin deficiency and yang deficiency and qi deficiency constitution,but there is no statistical difference between them in other constitution types.The statistical analysis of the differences of naming ability,attention,abstraction ability and orientation ability among different constitution types is not meaningful;4、There are statistical differences in naming ability scores among different gender groups(P=0.001),and women’s naming ability scores are lower than men’s.The difference of attention score and language ability score between different educational level groups is statistically significant,with attention p = 0.024 < 0.05 and language ability p = 0.041 < 0.05,and the scores of attention and language ability increase with the increase of educational level.Further multiple comparisons,the results of statistical analysis all suggest that:(1)there is significant difference between patients with education level of university(including junior college)and those with education level of primary school;The total score of Mo CA has statistical significance among different BMI groups(P=0.000).patients with BMI<18.5 and BMI≥28 have lower scores.further pairwise comparison shows that patients with BMI<18.5 have statistical differences with those with BMI≥18.5 and≤23.9 and those with BMI≥24and≤27.9,respectively The scores of Mo CA total score(P=0.001),language ability score(P=0.025),visual space and executive function score(P=0.036)were statistically significant among smoking groups,and all of them had lower scores.The naming ability score was significantly different between the drinking and drinking groups(P=0.043),and the drinking group scored higher.There are statistical differences in Mo CA total score(P=0.000),visual space and executive function score(P=0.019),abstract ability score(P=0.027)and delayed recall score(P=0.009)among the patients with or without hypertension,and those without hypertension have higher scores.The Mo CA total score(P=0.002),language ability score(P=0.006),delayed recall score(P=0.018)and orientation score(P=0.007)were significantly different among the diabetes patients,and those without diabetes had higher scores.The total score of Mo CA(P=0.002)and delayed recall score(P=0.007)were significantly different between groups with or without hyperlipidemia,and the scores of those without hyperlipidemia were higher.There was no statistical difference among other groups.Conclusion:1.The types of TCM constitutions with mild cognitive dysfunction were mainly Qi-deficiency,Yang-deficiency and Yin-deficiency;2.There were statistically significant differences in BMI,smoking history and diabetes mellitus among all types of mild cognitive impairment body types,while there were no statistically significant differences in age,gender,education level,drinking history,hypertension and hyperlipidemia between groups;3.The overall cognitive function of patients with mild cognitive impairment of yang deficiency and yin deficiency is worse.In terms of cognitive domain,the visual space and executive function and delayed memory damage of patients with yang deficiency are more prominent,the delayed memory damage of patients with yin deficiency is more prominent,and the language function damage of patients with damp heat is more prominent;4.MCI patients with low body mass index and obesity,MCI patients with a history of smoking and MCI patients complicated with hypertension,diabetes and hyperlipidemia have worse overall cognitive function.In terms of cognitive domain,the naming ability of female patients is more obviously impaired,the attention and language ability of patients with low education level are more seriously impaired,the visual space and executive function and language ability of MCI patients with smoking history are more seriously impaired,MCI patients with drinking history have less damage in the naming ability.In addition,the visual space and executive function,abstract ability and delayed recall were more seriously impaired in patients with hypertension,and the language ability,delayed recall and orientation ability were more seriously impaired in patients with diabetes,and delayed recall was more seriously impaired in patients with hyperlipidemia.
