| Migraine is one of clinical common primary headache, many had this disease in childhood and adolescence, and reach its peaks in the period of middle-age. There are more women than men, male to female ratio is about 1:2～3, prevalence rate is about 5%-10%, and the development of this disease has a common genetic background. Migraine is characterized by sex, throbbing headache, sustainable commonly 4 to 72 hours, and there can be with nausea, vomiting, optical, acoustic stimulus or daily activities can increase headaches, and associated with the degree of headache, the pain can ease with rest or quiet environment; There are some attacks with non-typical symptoms such as with or without visual field defect, eyes flashing, numbness or foreboding symptom such as dyskinesia. Migraine can be divided into six sub-types:without aura and migraine with aura, often for the precursor of children migraine periodic syndrome, retinal migraine, migraine six subtypes, complications, and among them, the migraine without aura is the most common type of clinical onset of migraine, which accounts for about 80% of migraine.The first part:literature researchObjective:A system evaluation of proprietary of DuLiang capsule and Flunarizine capsules in the treatment of migraine comprehensive curative effect and adverse reactions.Methods:Go through the domestic and foreign database use a retrieval strategy and from and manual inspection (the language is Chinese and English, do not consider any blind method), personnels qualified for inclusion and exclusion criteria of randomized controlled trials are independently selected, to evaluate the effectiveness of data of the literature included studies, regarding the Cochrane Reviewer’s faced 5.1 quality evaluation standards to take in literature, with RevMan5.2 software formeta analysis.Results:This study is divided into three parts, the total number of cases is 424, with 212 cases of treatment group and 212 cases of control group. Regrarding the clinical effectiveness, a total of 424 patients, the data show that there is a significant difference (RR= 0.75,95%CI (0.57,0.97], p= 0.03< 0.05) between the treatment group (DuLiang capsules group) and the control group (Flunarizine capsules group). Regarding the adverse reactions, three studies were included, two studies did not mention the occurrence of adverse reactions, only one study reported that the occurrence of adverse reactions.Conclusion:According to the existing evidence, the effect of DuLiang capsules treatment group is better than that of Flunarizine capsules group, and DuLiang capsules treatment group is with a low incidence of adverse reactions, too. Based on the limitations of this study, the strength of supporting evidence to be obtained, and the need to design rigorous, follow-up time longer confirmed by clinical research.The second part:the experimental studyObjective:This study used the proprietary Chinese medicine-DuLiang invigorate the circulation of T2DM has soft capsule intervention treatment and blood stasis resistance type brain collaterals in patients with migraine without aura, and from the clinical curative effect, Chinese medicine symptom curative effect and adverse reaction, compared with Xibiling than western medicine group study, to explore the clinical curative effect of invigorate the circulation of t2dm treatments for the disease, find certain scientific basis for the Chinese medicine treatment of migraine.Methods:60 patients were included in the test with the method of random number table is divided into experimental group and control group 30 cases each. Stop taking other medicines to treat have a headache more than 7 days, the Chinese medicine group oral DuLiang soft capsules, oral west than western medicine group Xibiling, treatment for 4weeks. Then based on the clinical curative effect, Chinese medicine symptom curative effect, adverse several aspects of the contrast research, assess the two groups of related symptoms and safety testing, summary of two groups of clinical trials data, sorting and statistical analysis.Results:The observation of the completion of this research project 2 years clinical cases 60 cases, the actual cases of 53 cases,26 patients of test group and control group in 27 cases, 7 cases of total loss cases. Migraine curative effect, treatment group total effective rate was 73.08%, the clinical recovery 0 cases;11 cases were markedly improved (42.31%),Effective 8 cases (30.77%),Invalid 7 cases (26.92%),Control group total effective rate was 81.48%, of which the clinical healing in 2 cases, accounting for 7.41%; 14 cases were markedly improved (51.85%),Effective 6 cases (22.22%),Invalid in 5 cases, accounting for 18.52%.By chi-square test, there was no significant difference in the two groups (P> 0.05), the treatment group and control group.TCM syndrome curative effect, treatment group total effective rate was 76.92%, including clinical recovery in 1 case,3.85%;10 cases were markedly improved (38.46%),Effective 9 cases (34.61%),Invalid 6 cases (23.08%),The control group total effective rate was 37.04%, including clinical cure 0 cases;4 cases were markedly improved (14.82%),Effective 6 cases (22.22%),Invalid 17 cases, accounting for 62.96%.By the statistical chi-square test, results the two groups have significant difference (P< 0.05), the experimental group in the traditional Chinese medicine syndrome curative effect is better than the control group. Adverse reactions, the treatment group 2 cases (7.41%) patients appeared dizziness, after the break of symptoms, without his lost processing, two groups of patients with other obvious adverse reaction was not found.Conclusion:The results of this study indicate that migraine DuLiang soft capsule treatment group is quite comprehensive curative effect and the west, Chinese medicine syndrome curative effect is superior to the west than Xibiling, with low incidence of adverse reactions. |
