Psoriasis is a chronic and easy relapsed erythematosus scaly skin disease. It’s a long-course desease and difficult to be cured, therefore, it severely influences the the patients’ quality of life, and so physical and mental health. Western medical treatment is deficient because of its toxicity and side effects. Traditional Chinese medicine has a long history and plenty of experience to treat psoriasis, which show certain advantages such as definite curative effect, few side effects and low recurrence rate.Fire needle therapy is one of the acupuncture treatment with characteristics of traditional Chinese medicine. It can warm and dredge collaterals, expel heat with heat, remove blood stasis, resolve toxins and hard masses by using a special needle to pierce in the certain parts of the body rapidly after heating with fire. Clinical studies shows that treating plaque psoriasis by fire needle therapy have a good therapeutic effect. "Huoxuejiedu decoction" is suitable for psoriasis patients with blood stasis syndrome, which formulated by the Chinese medicine dermatologist Zhao Bingnan. This decoction consists of peach kernel, curcuma rhizome, euonymus alatus, suberect spatholobus stem, oldenlandia, and so on, that have effects of invigorating blood, dissolving stasis and resolving toxins. It shows a satisfying curative effect for the treatment of psoriasis. In this study, a clinical observation was designed to evaluate the efficacy and safety of fire needle with Huoxuejiedu decoction.This thesis includes literature review and clinical research.Literature review:It consists of two parts. The first part is about the overview of plaque psoriasis in the treatment of troditional Chinese medine, which including the review of its historical names, symptoms, etiology, pathogenesis and treatment in ancient documents, the experience of treating plaque psoriasis by leading experts, and the progress of the clinical research on troditional Chinese medine in recent years. The second part is about the overview of psoriasis by acupuncture treatment, including the progress of acupuncture and moxibustion, bloodletting, auricular acupuncture, catgut embedment in acupoint, point injection, fire needle and sliding cupping therapy on clinical research in recent years.Clinical research:Objective:The program makes evaluations on efficacy and safety of fire needle with Huoxuejiedu decoction in the treatment of plaque psoriasis, then puts forward a safe, effective and propagable treatment scheme in traditional Chinese medicine of plaque psoriasis. Methods:By a randomized controlled method,52 patients with plaque psoriasis from May 2013 to July 2015 in Dongzhimen Hospital Affiliated to Beijing University of Chinese Medicine were evenly divided into experimental group and control group. In the control group, patients were treated by Huoxuejiedu decoction and Vaseline ointment, while in the experimental group, on the basis of treatment in the control group, fire needle therapy was provided. The period of treatment was 4 weeks. The PASI scores, TCM syndrome scores, itching index, area and symptoms scores of target region, and life quality scores were observed on these patients before and after treatment in both groups. All the data was analysed by SPSS17.0 software. The T test, rank sum test and chi-square test were used for the data respectively. Results:48 patients completed the survey,25 in experimental group and 23 in control group. There were no statistical differences in age, gender, course of disease, family history, PASI scores, TCM syndrome scores, itching index, area and symptoms scores of target region, and life quality scores before treatment(P>0.05). These two groups are comparable. The PASI scores, TCM syndrome scores, itching index, target region symptoms scores and life quality scores were significantly decrease in both groups after treatment(P<0.05). The target region area was significantly decrease in experimental group(P<0.05), while there was no statistical difference in control group(P>0.05). It showed that both groups had satisfactory effect on improving patients’symptoms, relieving itching and improving quality of life. The treatment of experimental group also had satisfactory effect on decreasing psoriasis area. The value of itching index in the experimental group was lower than that in the control group after treatment, which displayed a statistical difference (P<0.05). It showed that fire needle with Huoxuejiedu decoction was superior to only Huoxuejiedu decoction treatment on relieving itching. There were no statistical differences in PASI scores, TCM syndrome scores, area and symptoms scores of target region, and life quality scores after treatment(.P>0.05). The total effective rate was 92.00% in the experimental group and 82.61% in the control group, but there was no statistical difference in two groups(P>0.05). None of the patients in the two groups occurred with adverse reactions. Conclusion:Fire needle with Huoxuejiedu decoction is effective and safe in the treatment of plaque psoriasis, which is suitable to expand in clinical practice.
