Backgrounds & ObjectivesAIDS (AIDS) is a major diseases which serious health hazard to human life and socio-economic development. December 4,2009, UNAIDS released "2009 Update on the Global AIDS Epidemic" in Shanghai, shows that in 2009 the global AIDS virus (HIV) infection of about 33.4 million, killed 200 people. China since discovered the first case of AIDS patients from 1985, HIV infected and the number of case reports showing a rising trend, the cumulative report in 2009, shows that patients infected with HIV is about 32 million people, sexual contact accounted for 56%. Therefore, controlling the AIDS epidemic, we must begin to change the acts among the patients infected with HIV and those who have the risk behaviors to infected with HIV. Female sex workers (FCSW) is a risk factor for HIV transmission, because they are commercial sex transactions shortage of security protection measures, the continuous replacement of sexual partners and higher sexually transmitted disease (STD) prevalence. Once the population has HIV spread easily transmitted through unsafe sexual behavior to sexual partners, but also through those of partners spread to other people, and may be transmitted through sexual spread in the general population. The population of AIDS through sexual transmission plays an important role as a bridge to HIV transmission from high-risk groups to the general population, so it will be referred to as a bridge population. The female sex workers among who to behavioral interventions is the focus of the target population.In 1992,the first HIV infection was reported in Cixi.The cumulative report in 2009, shows that patients infected with HIV is about 112, The main transmission is through sexual contact. But so far, the city has not carried out in-depth research for FCSW behavior and effective behavior intervention research. Therefore, we carried out the behavioral monitoring and intervention studies, to understand the status of the city FCSW,to explore the effectiveness of interventions,to provide scientific evidence for which in order to further the work of high risk behavior intervention.Materials & Methods1. Selection of ObjectsCases of the study were the female commercial sexual workers(FCSW) who engaged in Entertainment in the city. Randomly selected entertainment,and continuous monitoring and intervention during 2008-2009.Totally, there were 148 patients. It were surveyed 447 in 2008,and 465 in 2009.2. Survey MethodsSpecialized questionaires were adopted, which were filled in by trained investigators after interviewing objects. The content of investigation included demographic characteristics, AIDS related knowledge, Access to sources of AIDS knowledge,Condom use and so on.Every quarter carried out comprehensive intervention, including health education, outreach services, promotion of condoms, promotion of peer education, promotion of proper medical treatment and so on. Developed 19 peer educators in order to sustain peer education. 3. Statistical analysisAfter questionnaires were coded unified, data were input into computers by Epi-data software, then database was set up, using Stata3.0 and SPSS13.0, Using descriptive study to analysis the general situation on the FCSW, prevent the spread of AIDS knowledge, and risk behaviors related cases.It analyzed the changes in the effects of interventions through the knowledge about AIDS before and after intervention, condom use to. It took out an integrated evaluation through comparing the peer educators, peer education and non-point data to analyze the different effects of peer education.Results1.The general situation of FCSW in our cityIt investigated 912 FCSW who were working at 48 Entertainment, including 447 monitored in 2008,the average age of whom were 22.92 (22.92±2.15); including 465 monitored in 2009, the average age of whom were 23.01 (23.01±2.26).Middle school educational level and below were accounted for most of Education. It was 331 cases in 2008,accounting for 74.05%;And 352 cases in 2009, accounting for 75.70%. Unmarried and Cohabitant cases were took the highest proportion of Marital status. In 2008 the Unmarried and Cohabitant cases were 210 and 167,accounting for 84.34%; and in 2009 those were 215 and 181, accounting for 85.16%. There were not Native of Cixi in Household situation, they were all floating population. In 2008,there were 75 cases in the province, accounting for 16.78%, and 372 cases outside the province, accounting for 83.22%. In 2009, there were 64 cases in the province, accounting for 13.76%, and 401 cases outside the province, accounting for 86.24%.2. Effect of comprehensive interventionThrough the implementation of comprehensive interventions, comparing the data of each group before and after intervention, the rate of correct answers of AIDS knowledge in 6 has increased, the differences were statistically significant. "Appearance can not see the infection" from 74.94% up to 83.87% during one year. "Dissemination of shared syringes" increased from 78.07% to 87.96%. "Condoms reduce the risk of transmission" increased from 77.18% to 84.03%, "Single sexual partners reduces the risk of infection" increased from 60.63% to70.11%, ""Having meal together do not spread" increased from 71.81% to81.93%,"Mosquito bites do not spread" increased from 45.19% to71.83%, the awareness of AIDS knowledge increased from 48.10% to 60.21%, the differences were statistically significant.3. Access to sources of AIDS knowledgeThe source of accepted knowledge mainly was television (88.05%), free promotional materials (70.50%). There are four ways significantly higher than before the intervention, they were statistically significant. To accept their friends up from 27.96% to 47.53%, Counseling services increased from 40.93% to 51.83%, TV channels increased from 85.01% to 90.98%, Free promotional material way from 66% to 74.84%.4. Effects of peer educationThe interventions carried out 19 peer educators to carry out sustained work, comparing the 19 peer educators, peer education and other non-point data point. According to Basic knowledge about AIDS, peer educators point are significantly higher than non-peer education point about the correct rate of 5 question. "Appearance can not be seen"(88.10% and 80.39%, P<0.05), Dissemination of shared syringes (93.33% and 83.50%, P<0.01), "Reduce the risk of a single partner" (76.67% and 64.71%, P<0.01),"Having meal together do not spread" (88.10% and 76.86%, P <0.01),"Mosquito bites do not spread" (80.48% and 64.71%, P<0.01).Peer education point slightly higher than the point of non-peer education in AIDS Awareness (65.71% and 55.69%, P<0.05).Peer education and peer education point of point and non-significant difference in condom use. ConclusionThe study conducted on 912 FCSW behavior and health knowledge to monitor the implementation of comprehensive interventions, and effective development of the 19 peer educators. Study found that it was little effect to raise FCSW knowledge and to change behavior only rely on behavioral monitoring and public awareness. Health education, health counseling, distribution of free information, outreach services and other interventions in FCSW, can improve the prevention and treatment of AIDS Awareness. Knowledge of change to some extent led to a change in behavior. Condom use in commercial sex behaviors was also higher. Compared to other interventions, peer education can play better results. Through the implementation of peer education, AIDS knowledge level and condom use increased more significantly. We further need to study the contents were how to find, develop peer educators, good communication, persuasion, coordination, standard content of peer education work.Keywords:AIDS; Female sex workers; Interventions; Peer Education...
