Impotence is also named Erectile Dysfunction in western medicine. There are lotsof records about impotence in ancient books of TCM and also in modern medicalmagazines. Impotence is one of the most frequent diseases in male sexual dysfunction.Because of a high incidence rate, it influences the living qualities of patients badly andcauses a series of problems in family and society, and gradually it is recognized bydomestic and international specialists in the medical field. The reports of relatedresearch are abundant; but there are lots of different recognitions theoretically. Therelevant syndrome differentiation standard is not quite normative, and related clinicalreports usually are trials with low quality, the re-usable rate is low. This paper consists of three parts which is related to impotence. It discusses thenormative syndrome differentiation standard for impotence from the aspects oftheoretical study, systematic review and clinical practice.Theoretical Studies: This part consists of two parts: the definition, standards for differentiatingdiseases and the cognitions in the history. The name impotence has a long history, although it has a single same name,"YangWei" in Chinese, the definition and standards for differentiating diseases aredifferent. The cognitions in history involve the evolvement of definition, theunderstanding of pathogenesis and cause of the disease, the syndrome differentiationand therapy. We trim and summarize the theories about impotence in history, andintroduce those creationary and unique theories.Systematic Review: EBM Meta-analysis are used to evaluate the RCTs of treating impotence withTCM or integrated traditional Chinese and western medicine. We search all the articlesabout clinical trials from 1994 to 2003 and find 4 of them are accord with the need; allarticles found are trials with low quality. This SR shows that, compare with treatmentof western medicine, the curative effect of those treated with TCM are with significantdifference, these therapeutics can not be recommended to treat impotence according toevidences in existence due to the potential publish bias, measure bias and defect ofdesign.Clinical Studies:1.Clinical Epidemiology Study for Impotence6 阳痿病中医证候规范化及计量诊断研究 According to the survey based on 293 impotent patients, the epidemiologyfeatures of syndrome and other aspects interrelated is analyzed. This survey shows the probability of impotence increases with the age before 40years old, which reaches the high peak in the age class above 40 and proved thedefinition that impotence is a kind of aging diseases. The number of patients in the ageclass of 40 to 50, 50 to 60 and 60 to 70 decreases gradually, which might be caused bythe low hospitalizing rate of aging patients. To all patients, the incidence among cadreis the highest one, the next is the worker's and the intellectual's, the farmer's and thejobless ones are the least. But the survey is not representative. The degree ofimpotence aggravates with aging, especially the frequency of the serious case. Most symptoms of impotence are related to liver and kidney, but not related to thetwo viscera only. All the symptoms of heart, liver, spleen and kidney can occur, or thatall associated with each other. So we can conclude that impotence is relative to manyviscera extensively, and the single syndrome is not typical. Syndrome of liver-qistagnation and syndrome of kidney-yang insufficiency are two primary syndromes,syndrome of yang deficiency in the spleen and the kidney is another important one.The primary pathogenesis are liver-qi stagnation, kidney-yang insufficiency,damp-heat and blood stasis.2 .
