| Heart failure belongs to the categories of "heart paralysis","heart palpitations","Zhengchong","Asthma","edema","phlegm" and "Water" in TCM, with high morbidity and high rate of deformity, is an important disease that effects the health of human in the new century. Foreign statistics show that the morbidity of HF is about 1.5%to 2.0%, for the people who are at the age of 65, the rate can reach 6%to 10%. Over the past 40 years, the death of HF increased six times. The cooperate study of Chinese cardiovascular health centers in China have extracted large samples of representation since 2000, the results show that the incidence of adult heart failure was 0.9%, 0.7%in males and females 1.0%(P<0.05); The incidence of HF rises significantly with the increase of age. In recent years many relevant clinical studies also found that coronary heart disease is the first cause in heart failure, and infection in chronic heart failure is an important cause of acute decompensated heart failure. For the patients with acute decompensated heart failure, the clinical medicine treatment is primary, what's more, hemodynamics and the improvement of symptoms are the goals of treatment in the recent. Chinese medicine in acute decompensated heart failure is mainly used in the current pulse injection and possessed of injection in proprietary injection, etc, and in most cases, it's lack of the dialectical guide of TCM theory, while the usage of Chinese medicine's traditional decoction is very limited. This research is mainly for the patients who have acute decompensated heart failure, these people are used dialectical system of Chinese medicine with inspection, auscultation and olfaction, interrogation and palpation, with the combination of western medicine treatment. According to "The Clinical Research Guidance for Chinese herbal medicine's treatment of HF" in "The guide principles of Chinese herbal medicine's clinical research", which was published in the year of 2002, as diagnose standards to be selected for cases typification and prescription drugs, so formed treatment programs of Chinese and western medicine. Compared to pure treatment group, observe the acute phase of hemodynamic and improvement of symptoms of TCM, it's a exploration for the position and function of acute decompensated heart failure in the clinical diagnosis.This paper is divided into two parts, the first part is a document research of two reviews. There are some summaries for doctrines of various historical schools and progress of modern research from heart failure's syndrome recorded history to etiology and pathogenesis in "Chinese Medicine's Dialectical Treatment Research of Heart Failure". There are detailed discussion from the definition of HF and epidemiology to modern etiology cognizance and severe classification standard, especially the definition and diagnosis of acute decompensated heart failure and medication progress etc. in The Modern Knowledge and Diagnostic Progress of Acute Decompensated Heart Failure. The second part is the clinical research, the purpose of this study is to meet the standard of heart failure symptom's severity quantification and the improved level of NYHA's classification. This paper discusses the improvement of acute decompensated heart failure symptoms with the method of reinfore qi, invigolate blood and promoting diuresis, through observation of hemodynamic and clinical symptoms for the acute decompensated heart failure. This study observed 35 clinical cases,22 cases of experimental group and 13 cases of contrapositive group. Experimental group cures with integrated Chinese and western medicine treatment, contrapositive group cures with simple western medicine treatment, and what' s more, immediate admission and the improvement of symptoms and heart function on the fifth day are recorded in detail, statistically analyzed the two groups at last. The results show that the group with the method of reinfore qi, invigolate blood and promoting diuresis and western medicine are better than pure western treatment group in the aspect of the improvement of patients'acute congestive symptoms and left ventricular ejection fraction (P< 0.05). The two groups are significantly different. But there are no difference between the two groups in NYHA's classification and Lee's failure score. While according to the statistical data, the experimental group has more higher trend than contrapositive group in efficient respects of all medical standards. This proves that the method of reinfore qi, invigolate blood and promoting diuresis with western medicine treatment's scheme is superior to pure western medicine in the treatment of acute decompensated heart failure, especially in the aspect of improvement of acute symptoms. |
