OBJECTIVETo observe the influence of Dexmedetomidine on the recovery time (respiratory recovery time, the awaking time, the orientation recovery time, the awaking extubation time), the hemodynamics(such as heart rate and blood pressure), Blood oxygen saturation(SpO2),the respiratory rate and the complications in the waking up period (postoperative restlessness,omitting and shivering) of patients who were suffering abdominal hysterectomy with general anesthesia.METHODS1.To collect sixty-four patients(ASAⅠ-Ⅱ) of Gynecology of our hospital (Shandong Provincial Hospital).The patients ranged in age from 33 to 58 years old and in body weight from 48 to 72 kg.2.General anesthesia were used in all the operation.The 64 patients were randomly divided into two groups(experiment group, control group, n=32).The patients of experiment group received intravenous dexmedetomidine (Jiangsu Hengrui Medicine Co., Ltd.) infusion with the load dosage which is 0.5 ug/kg finishing in 5 minutes,at a speed of 0.02ug/kg/h intravenous dexmedetomidine in the operation, and the patients of control B group received intravenous normal saline infusion in the same way. The patients in both groups received penehyclidine hydrochloride,midazolam,fentanyl,etomidate,atracurium esmolol Hydrochlorid for anaesthesia induction.The patients in both groups received Propofol. atracurium for anaesthesia maintenance.3.By use of the Narcotrend anesthesia monitoring system to observe the anesthesia recovery time(the respiratory recovery time, the awaking time, the orientation recovery time, the awaking extubation time) and the blood pressure (BP), heart rate (HR),Blood oxygen saturation(SpO2)before and after the awakening extubation process.The incidence of postoperative restlessness,omitting and shivering.RESULTS1.the fundamental state:There is no differences between patients of the two groups in age,body weight or the operation time. (P>0.05)2.Anesthesia recovery time:(1) The respiratory recovery time:the respiratory recovery time of patients in group A is shorter than that of patients in group B(group A:15±5.1 min; group B:20±7.0min, P<0.05);(2)the awaking time: the awaking time of patients in group A is shorter than that of patients in group B(group A:20±3.5min; group B:26±5.8min, P<0.05);(3)the orientation recovery time:the orientation recovery time of patients in group A is shorter than that of patients in group B(group A:24±3.6min;group B:32±5.5min,P<0.05);(4)the awaking extubation time:the awaking extubation time of patients in group A is shorter than that of patients in group B(group A:26±3.8min, group B:35±6.5min, P<0.05).3.The change of hemodynamics:(1) The change of blood pressure of patients in group A is smaller than that of patients in group B during awaking process. (P<0.05).(2)In extubation there is one person with hypertension in group A (3.13%),while five patients in group B (15.6%)4.The complication:(1)There is 1 patient omitting in group A,while 3 patients in group B.The incidence of group A is smaller than that of group B (P<0.05);(2)There is 1 patient with shivering after operation in group A,while 4 patients in group B.The incidence of group A is smaller than that of group B (P<0.05).;(3):1 patient has postoperative restlessness in group A (3.13%),while 4 patients in group B (12.5%).The incidence of group A is smaller than that of group B (P<0.05)5.Blood oxygen saturation of the two groups are both in the normal range, no statistically significant. (P>0.05)CONCLUSIONThe sedative effect of Dexmedetomidine is very good. It can shorten the anesthesia recovery time and awakening extubation process. And it can keep the hemodynamic stable during the process of extubation.Meanwhile, it can reduce the incidence of postoperative restlessness, not increasing the respiratory depression.
