BACKGROUND Malignant ascites is defined as abnormal accumulation of fluid in the peritoneal cavity as a consequence of malignant tumor in abdominal cavity, which happens frequently in lacteal gland, bronchus, ovary, stomach, pancreatic gland and so on. It severely affects the quality of life of patients. The previous treatments consist of diuresis, tapping by abdominal paracentesis repeatedly, and so on. But the curative effect is below the mark. During the past 10 years, CCCHP(Coelom Continued Circulatory Hyperthermia Perfusion) as a new method in curing peritoneum metastasis of malignant tumor in abdominal cavity, achieves a better curative effect, which is applied more and more generally at home and abroad. However, the cognition of CCCHP is not sufficient, so the further investigation is necessary for safe and effective clinical treatment, especially the safe therapeutic condition is still controversial. And it is lack in experimental data and reports on thermal injury of large animals. To find the optimum temperature and other optimum conditions in CCCHP, the canine mode and rats with walker256 ascites tumor are established in this study, which provides experimental evidence for the clinical treatment of malignant ascites by CCCHP.OBJECTIVE 1,To investigate the impact of Coelom Continued Circulatory Hyperthermia Perfusion (CCCHP) by TRL type hyperthermic intraperitoneal treatment system with different temperature and treatment conditions on experimental animals vital signs and important organs pathology. Further to search optimal temperature and the way to control intraperitoneal perfusion temperature effectually.2,To investigate the effects of rats with walker-256 ascites tumor after taking Coelom Continued Circulatory Hyperthermia Perfusion (CCCHP) under different temperatures. To explore the safe and effective temperature combining together the results of the first part of the experiment. METHODS 1,Sixteen medical fitness mongrel dogs were experimented with TRL type hyperthermic intraperitoneal treatment system at 41℃(normal saline group and DDP group), 42℃(normal saline group) and 43℃(normal saline group) intraperitoneal temperature. CCCHP was performed three times(one hours per time). During CCCHP, The dogs'vital signs and instrument parameters were recorded. The animals hepatic and kidney function changes of CCCHP were inspected before the course and after the last CCCHP by preserved blood specime. The general morphological changes and pathological changes of abdominal organs were inspected after the administration 24 hours and 2 weeks.2,Inoculate walker-256 cell into abdominal cavity of SD rats and establish malignant ascites model. Forty SD rats were fractionated into four groups randomly. CHIPC groups(A and B group): the rats in A group were experimented at 41℃with DDP, the rats in B group were experimented at 42℃with DDP; the intraperitoneal chemotherapy group(C group): take suction the ascites 50ml and inject DDP 5mg/kg dissolving saline at 38℃; control group: do not take any treatment. The groups with treatments(A, B and C group)were cured omnibus diebus quaternis. CHIPC was performed three times(one hours per time). Observe the mean survival time of rats in every group, inspect the general morphological changes and pathological changes of abdominal organs were ed after the rats were died and the tumor tubercles were observed with light microscope and electron microscope.RESULTS 1,Single CCCHP 41℃three times has no obvious effects on vital signs and hepatic, renal functions of experiment animals. The liver, kidney, spleen and intestine have only slightly pathological impairments after CCCHP, and which can quickly recover. CCCHP combined with DDP chemotherapy 41℃three times has no obvious effects on normal physiologic function. CCCHP at 42℃and 43℃three times have significant effects to hepatic and renal functions of experiment animals, and liver, kidney, spleen and intestine have significant pathological impairments in different degree after CCCHP. The impairments aggravated with increasing temperature.2,The mean survival time of the rats in CCCHP groups extended obviously than the intraperitoneal chemotherapy group and the control group (P <0.01), and there was significant difference between the intraperitoneal chemotherapy group and control group (P <0.05). There was no significant difference between 41℃CCCHP group and 42℃CCCHP group (P >0.05), and there was no obvious difference of the degree of organ injuries between the two groups. CONCLUSION 1,For dogs, 41℃intraperitoneal temperature CCCHP three times (one hours per time) is safe and feasible, so it can be choose as conventional thermochemotherapy temperature with CCCHP.2,Coelom Continued Circulatory Hyperthermia Perfusion has conspicuous antitumous effect and the mean survival time of the rats in CCCHP groups extended obviously than the intraperitoneal chemotherapy group and the control group.3,The mean survival time of rats in 42℃CCCHP group was longer than 41℃CCCHP group, but there was no statistically significance(P >0.05). There was no obvious difference of the degree of organ injuries between the two groups, and liver, kidney, spleen have slight impairments.
