With the social and economic development, the pace of life accelerating, sense of competition increasing, as well as life habits changing, human health faces greater challenges, sub-health population is gradually increasing. That the sub-health is the"preclude" of diseases, has been referred to one of the greatest threat to human health in 21st century by the medical scientists. If we can take timely and effective interventions, from the health management, psychological and social dimension etc, it may prevent the sub-health to chronic disease, and then reduce the occurrence of various chronic diseases, lighten the burden of chronic diseases. Therefore, this research has important the oretical and practical significance.ObjectiveTo investigate the sub-health status of civil servants in Shenyang; to explor and investigate the related factors that affect the sub-health of civil servants in Shenyang; and to bring forward the corresponding management strategies to provide reference for the improvement of the health status of civil servants in Shenyang.MethodsBy stratified cluster sampling,2700 male and female civil servants with a mean age of 37.66±9.802 years of urban area in Shenyang were investigated during the period from July to September,2010. Among 2547 collected questionnaires,2412 were effective with an effective rate of 94.70%. There were 668 classified integrated management institutions of politics and administration; 592 classified integrated management institutions of finance and economy; 579 classified management institutions of financial and economic industry; 564 classified integrated management institutions of science, education, culture and health; 9 classified other agencies.The questionnaire was composed of Sub-Health Measurement Scale Version 1.0 (SHMS V1.0).All data were input into computer to establish a database using EpiData3.02 and processed using SPSS13.0 software package. The statistical methods were descriptive statistic,t-test, one-way ANOVA, and regression analysis, et al.Result1. The Analysis of the sub-health status of civil servants in Shenyang1) The standard mean of physiological, psychological and social subscal of SHMS V1.0 among civil servants in Shenyang respectively were 67.41±14.331, 66.61±16.276,61.28±17.588. And their total mean scores were 65.99±13.769.2) Using dimension analysis, SHMS V1.0 dimension with the mean score being lower than 73 among civil servants in Shenyang were sorted out. It was found that the low dimensions in subscale of physical symptoms, organ function, energy, positive emotional, psychological symptoms, cognitive function, social adaptation, social resources and support.3) Using item analysis, it was found that the low score items in subscale of physical health reflected gastrointestinal symptoms, fatigue; the low scores of the items in the psychological subscale of SHMS V1.0 were positive feeling, and memory and concentration; the low scores of the items in the social subscale of SHMS V1.0 were relationships, seeking help from others.2. Analysis on one-factor that affects the sub-health of civil servants in Shenyang.1) There was significant difference in subscale, scores in physical sub-health, psychological sub-health and allover sub-health of when the subjects were divided into groups according to their ages(P<0.05), but the scores in factors of social sub-health had no significant difference. Scores in physical sub-health and social sub-health when the subjects were divided into groups according to their Body Mass Index(P<0.05), but the scores in factors of psychological sub-health and allover sub-health had no significant difference.2) The scores of the subscales and the overall scores of the sub-health of civil servants in Shenyang show significant statistical difference (P<0.05), in the aspects of frequently having breakfast, taking physical exercises and smoking or not. There is significant statistical difference in the aspects of having enough sleeping time, using different traffic tools and holding different views on pollution(P<0.05). The scores in drinking or not of social sub-health and overall sub-health had no significant difference.3) It could be found from the status of sickness influencing sub-health that, the scores of the unaffected were higher than that of the affected on the physical sub-health, psychological sub-health, social sub-health and allover sub-health of SHMS V1.0(P<0.05); the scores of the affected who achieved steady controlling were higher than that of the poorly controlled in every domain(P<0.05).4) It could be found from stressful life events influencing sub-health that, the scores of coming through stressful life events about family discord, family financial problems and the tension between colleagues were lower than that the ones who did not experience these events mentioned above(P<0.05); stressful life events of housing problem influenced the score of psychological subscale and overall score of the SHMS V1.0(P<0.05); stressful life events of high pressure from work influenced the score of physical subscale(P<0.05); stressful life events of retirement had no significant difference in every subscales'scores and overall score of the SHMS V1.0.3. Multiple stepwise regression analysis of the factors influencing the sub-health of civil servants in Shenyang1) Mutiple stepwise regression analysis is conducted in the research. The dependent variables are the scores of the subscales and the overall score of the SHMS V1.0 of civil servants in Shenyang, while the independent variables are sociological characteristics(gender,age,nationality), life style, sickness or not, diseases controlling and stressful life events. When a=0.05, the conditions of having breakfast and taking physical exercises are found in all four regression equations, but the six factor scores of sociological characteristics, four factor scores of lifestyle, two factor scores of the situation about sickness and diseases controlling situation, and three factor scores of life events are found in different regression equations.2) Ordinal logistic regression analysis was used to study the influential factors of sub-health of civil servants in Shenyang. The dependent variables are the different conditions of body, including health status, sub-health status and disease status. The independent variables are sociological characteristics (gender,age,nationality), life style, sickness or not, diseases controlling situation and stressful life events. Six influencing factors of sub-health were seniority, holding different views on pollution, smoking or not, drinking or not, family average income per month, job grades and sex.Conclusions1. The scores in four domains of sub-health of civil servants in Shenyang from sixty to seventy-three. Their scores of physical, psychological and social domain of SHMS V1.0 suggested that they stationed in moderate sub-health condition. The main problems disturbing the state of physical sub-health among civil servants in Shenyang include gastrointestinal symptoms, fatigue, and insomnia; the main symptoms of anxiety, tension, obsession, depression, loneliness, and poor memory in subscale of psychological sub-health. The main questions of interpersonal relationship and seeking help from fiends while facing difficult situation in subscale of social sub-health.2. The symptom of sub-health in male civil servants was obvious, and the symptom of sub-health in the retired was obvious too; the symptom of sub-health in the old was obvious, and the symptom of sub-health in the civil servants who stationed in high-level ranks was obvious too; the health status of those civil servants in Shenyang who engaged in simple work and work in the unites of high-level was in good condition; the sub-health symptom of those civil servants in Shenyang who frequently smoke, drink, have little sleeping time, lack of exercises and didnot have breakfast was quite obvious.3. Stressful life events directly affect sub-health of civil servants in Shenyang.4. It could be found from the ordinal logistic regression analysis that, six influencing factors of sub-health were seniority, holding different views on pollution, smoking or not, drinking or not, family average income per month, job grades and sex.5. The status of sub-health among civil servants could be improved by the 5A' measures, such as developing health surveies, making health plans, implementing health intervention measures and implementation of health assessment. The health management that arouse governments, communities, health insurance companies and medical institutions, medical personnel and patients that almost everyone participated was the philosophy of development about health management. The results suggests that more attentions should be highly paid to the health conditions of civil servants and corresponding measures be taken actively in many aspects in order to improve their overall level of health and quality of life. And then enhancing the battle effectiveness of the forces.Characteristics and Innovation1. This is a systematic and thorough investigation on the sub-health status of civil servants in Shenyang to explore the impact of sociological characteristics, lifestyle, sickness or not, the situation of disease controlling, and stressful life events on their sub-health. With a large sample size, the results of the research are highly representative.2. The research quantitatively reflects the overall level of the sub-health status of civil servants in Shenyang, and their physical, psychological, and social sub-health. It expores and sums up the factores that influence their disease state, sub-health state, and health state.3. The research pertinently comes up health demand management strategies and integrated management strategies for improving the health status of civil servants.Limitations and Implications1. As a cross-section survey, the limited results of the research need further experimental verification.2. The subjects of this study are those civil servants in Shenyang. In the following studies, sub-health status in the other cities of civil servants in Liaoning province should be included.
