Objective:To comparision of the remifential and isoflurane anesthetic techniques on liver function in patients undergoing liver cancer.Methods : Forty ASAⅠ-Ⅱpatients of liver cancer scheduled for elective hepatectomy were enrolled and randomly divided into two groups: group remifentanil (group R) and group isoflurane(group I). Group R were induced and maintained with remifentanil and propofol, while group I with isoflurane and propofol. Hemadynamics changing during the induction and maintenance phase, spontaneous breath recovery time, awake time, time of endotracheal tube extubation and side-effect were investigated and compared. The liver function was tested at the beginning of the operation ( D0 ),1day after operation ( D1 ),3 days after operation ( D2 ),5days after operation ( D3 ), 7days af ter operation( D4 ).Results:1.MAP during drag organs was significantly higher as compared with preoperative (P<0.05) in group I, while hemadynamics changing were no significantly differences in group R. 2.In group R,spontaneous breath recovery time, awake time, time of endotracheal tube extubation were shorter than group I(P<0.05),and the score of OAAS(sedation scores after tracheal extubation) was higher than group I. 3.There was obvious increased in the serum concentration of ALT, AST at D1 in both groups, but less in group R than in group I( P < 0. 01). The serum concent ration of ALT, AST came back to normal at D2~D3 in groupⅠ, but at D4 in group I. The serum concentration of TBIL was increased at T1 in both groups , but less in group R than in group I( P < 0. 05) . And it returned to normal at T4 in both groups( P < 0. 05).Conclusion: Remifentanil anesthesia resulted in more protection for liver function, less circulatory disturbance, faster recovery and less post-operative complications,so remifentanil can be used more safely and effectively in hepatectomy for Liver Cancer.
