| Part One Theory ResearchThe major content of this part is the review on advances in myofascial pain syndrome:Under the guidance of basic TCM theory, literature reports related to myofascial pain syndrome in recent years are systemically reviewed, primarily including etiology theory of myofascial pain syndrome, its relative experimental researches, the comparison with TCM meridian-collateral theory and systemic summarization on treatment methods. The purpose of the dissertation is to comprehensively explore the theoretical basis involved in the studies of myofascial pain syndrome on macroscopic and microscopic levels in order to provide theoretical and technical support for the researches on acupotomy in the treatment of myofascial pain syndrome.Part Two Animal ExperimentThe major content of this part can be summarized in the following:Objective:To observe the histological variations of skeletal muscle and fascia cut by acupotomy in fixed sites below the femoral greater trochanter of normal mice in order to provide morphological data for the fundamental mechanism research on acupotomy medical therapeutics. Methods:26mice were divided into blank control group, acupotomy-intervened group one, acupotomy-intervened group two, acupotomy-intervened group three and acupotomy-intervened group four, in which there were2mice in blank control group and6in other acupotomy-intervened groups. Pathological hematoxylin and eosin (HE) routine staining in blank control group, acupotomy-intervened group one (1d after acupotomy intervened), acupotomy-intervened group two (2d after acupotomy intervened), acupotomy-intervened group three (5d after acupotomy intervened) and acupotomy-intervened group four (lOd after acupotomy intervened) was adopted to observe tissue micro-structure changes of skeletal muscle and fascia in each group. Results:By HE staining, it was found that normal skeletal muscle and fascia tissue in fixed sites below the femoral greater trochanter of the mice intervened by acupotomy presented a sort of reactive inflammatory change with blood vessels centered and macrophage aggregation, afterwards, a series of reparative processes occurred and the reparative results had a tendency of normal skeletal muscle and fascial structures. Conclusion:After normal muscle tissue of mice is intervened by acupotomy, the rules of their micro-structure changes are a process from reactive inflammatory changes by acupotomy to gene modulation, then to tissue reconstruction, namely the tissue cut by acupotomy can directionally repair and reconstruct under gene modulation, which provides morphological evidence for researches on the fundamental mechanism of acupotomy therapeutics and has positive significance.Part Three Clinical ResearchThe main content of this part can be summarized as follows:Objective:To observe the clinical effects by the combination of acupotomy and kneading patella in the treatment of62patients with knee osteoarthritis and investigate the mechanism of acupotomy for treating diseases. Methods:Acupotomy was used to loosen the tender points of lesions in peripheral knee muscle, and kneading patella to assist the knees after loosening, once in7days and3times as one course of treatment. Usually, the treatment should be one to two courses. Results:After one course of treatment, the efficacy assessment on79affected knees in selected62patients was26excellent knees,41good knees,11common knees and1bad knee, in which the excellent rate and total effective rate were84.8%and98.7%respectively. Conclusion:The combination of acupotomy loosening and kneading patella has better clinical effects for treating knee osteoarthritis, which reveals reparative and reconstructive mechanisms of the lesion tissue cut by acupotomy and provides clinical experience for acupotomy in the treatment of myofascial pain syndrome.ConclusionsImportant conclusions of this study are as follows:1. The mechanism of acupotomy for treating myofascial pain syndrome is that the connective tissue in lesion part produces a large number of macrophages after being cut by acupotomy, leading to cell fragments, necrotic tissue and a small amount of bacteria being swallowed and activation of regenerative stem cells between sarcolemma and basement membrane by secreted growth factors, which improve local ischemic and anoxic state, promote re-repair and reconstruction of injured muscle tissue and make the repair have a tendency of normal structures.2. For treating myofascial pain syndrome, acupotomy has the advantages of rapid efficacy, short duration and high recovery rate. It deserves to be popularized and applied in clinic. |
