Objective:To observe the curative effect evaluation of the treatment of knee osteoarthritis through simulated acupotomy approach based on the Jingjin theory,and to provide theoretical basis and research ideas for the formulation of precise treatment and standardized operation based on the Jingjin theory.Methods:From December 2020 to December 2021,70 cases of KOA patients with grade I to III who accorded with the diagnostic criteria and the research inclusion criteria of KOA were selected from the outpatient or inpatients of the Department of Orthopedics and Traumatology of the People’s Hospital Affiliated of Fujian University of Traditional Chinese Medicine.According to the random number table method,70 KOA patients were randomly divided into test group and control group,35 cases in each group.The test group was treated with acupotomy along clear acupotomy penetration angle and depth,and the control group was treated with acupotomy along four rules of routine.The treatment points in both groups were“Hedingci”,“Binneixia”,“Binwaixia”,“Yinlingshang”.The acupotomy treatment is performed once a week,4 times as a course of treatment,and a total of a course of treatment.The changes in observational indicators at three points in time(pre-treatment,1 week after treatment,3 months after treatment)were recorded,such as the WOMAC scores(pain score,joint stiffness score,daily ctivity score,and comprehensive score),the range of motion of the knee joint and ultrasonography(elastic strain rate of rectus femoris muscle and joint fluid depth of the suprapatellar bursa).SPSS 20.0 software is used to organize research data and statistical analysis.Results:1.In general data,the baseline level is consistent and comparable.2.WOMAC score:(1)Pain score,the scores of both groups were lower after treatment and at follow-up visit than before treatment(P < 0.05).Both groups could effectively relieve the pain of patients with KOA,The scores of the test group were lower than those of the control group after treatment and during follow-up visit(P < 0.05),and the test group was superior to the control group in pain relief.(2)The scores of joint stiffness and daily activities in both groups were lower after treatment and at follow-up visit than before(P < 0.05).Both groups could effectively improve the joint stiffness and promote the recovery of daily activities in KOA patients.After treatment,the test group was better than the control group in improving joint stiffness and resuming daily activities(P < 0.05),and the efficacy of the two groups was similar at follow-up visit(P > 0.05).(3)The total scores of the two groups were lower after treatment and at follow-up visit than before(P < 0.05),and the knee function of KOA patients was effectively improved in both groups.The score of the test group was lower than that of the control group after treatment and follow-up visit(P < 0.05),and the test group was superior to the control group in the comprehensive score.3.Rrange of motion of the knee joint:The range of motion of the lower extremities in both groups was significantly increased after treatment and during follow-up visit(P < 0.05),and the range of motion of the lower extremities was effectively improved in both groups.The activity of the test group was better after treatment(P < 0.05),but the activity of the control group was better during follow-up visit(P < 0.05).4.Ultrasonography: The elastic strain rate of rectus femoris muscle in both groups was effectively reduced after treatment and during follow-up visit(P < 0.05),and the test group was superior to the control group after treatment and follow-up visit(P < 0.05).The joint fluid depth of the suprapatellar bursa after treatment and during follow-up visit,joint effusion was effectively reduced in the two groups(P < 0.05),and the reduction degree in the test group was better than that in the control group(P < 0.05).Conclusions:Acupotomy has clinical efficacy in improving symptoms and signs as well as ultrasound images.However,based on the theory of acupotomy,acupotomy along clear penetration angle,depth and release range is superior to traditional acupotomy in reducing pain,reducing muscle hardness and reducing joint effusion.
